All travelers enteringGuamunder the termsof theGuam-CNMIVisa
WaiverProgramare required to completean I-94Arrival/Departure
Record (oneper person, including infants); an I-736 (oneper person,
including infants); andaGuamCustomsDeclaration (oneper family).
All other travelersneedonly completeaGuamCustomsDeclaration.
All formsmust be completed inEnglish, in capital letters. Be sure to
include the street nameandnumber, cityand stateof your address in
Guam. If youare transiting throughGuam, youmaywriteTRANSITand
your final destination country. TheCustoms andBorderProtectionofficer
will place the I-94DepartureRecord inyour passport after inspection.
Make sureyou return theDepartureRecord to theairline representative
beforeboardingyour return flight.

ExpeditedPassport Control andCustoms
Clearance in theU.S.—Global Entry™
CBPoffers theGlobal Entry™program inorder to
expedite theprocessingof pre-approved, low-risk
international travelers entering theU.S. Pleasenote thatGlobal Entry is
not available to travelersarrivingatHonoluludirectly fromGuam. Upon
returning from travel abroad, travelers enrolled inGlobal Entrymay
bypass the regular passport control line andproceed to theGlobal Entry
kiosk. Global Entryprogramparticipants scan theirmachine-readable
passport, U.S. permanent resident cardorU.S. visaon the kiosk, place
their fingertips on the scanner for fingerprint verification andmake a
customs declaration. The kioskwill issue a transaction receipt anddirect
the traveler tobaggage claim and exit. Kiosks are located atmajorU.S.
airports, aswell as at several CBPPre-Clearance locations.
The following travelers are eligible for enrollment inGlobal Entry:
• Citizens and residents of theU.S.
• Citizens of Germanywhohavepreregisteredwith theGermanFederal Police
• Citizens ofMexicowhohold aU.S. visa
• Citizens of theNetherlandswho are enrolled inPrivium
• Citizens of Panamawhohold aU.S. visa
• Citizens of SouthKoreawho are enrolled in SES (Smart Entry Service)
•Members ofNEXUSor SENTRI
Application for enrollment inGlobal Entry™ is available at theGlobal
On-LineEnrollment System: goes-app.cbp.dhs.gov. It costs only$100,
which covers enrollment for a five-year period. The governmentwill
review the applicant’s informationwhile abackground investigation
is conducted. Applicants undergo an interviewwithCBPofficers at an
Enrollment Center in theU.S. before final approval is granted.
All Global Entrymembers are eligible toparticipate in the
® program.

expedited screening through TSA checkpoints at several airports.

Expeditedscreening through theU.S. TransportationSecurityAdministration (TSA)
is a TSA-managed and –operated
expedited screening initiative available inmany
U.S. airports. Benefitsmay includeno longer
removing the following itemswhen going
through airport security: shoes, light outerwear/
jacket, belt, 3-1-1–compliant bag from carry-on
and laptop frombag.

TheTSAuses randomandunpredictable secu-
ritymeasures todetermine customereligibility
forexpedited screeningonaper-flightbasis.
Therefore, youarenot guaranteedexpedited
screening forevery flight even ifyouhave
applied to theprogram.
MileagePlusmembersmayparticipateby
joining aDHS Trusted Traveler program. Visit
tsa.gov/tsa-precheck to learnmore.
ApprovedGlobal Entry/NEXUS/SENTRI
members receive amembership/PASS ID
number, also called aKnown TravelerNumber
(KTN). Approved
Application
Programmembers receive aKTN.
Members of theU.S.Armed Forcesmay use
their DoD IDnumber as aKTN. Enter the
KTN intoyourMileagePlus profile at
united.com/tsaprescreening. Unitedwill
transmit theKTN to the TSA alongwith
the Secure Flight Passenger Data inyour
reservation so the TSA candetermine your
eligibility for
.
If the TSAdetermines a passenger is eligible
for expedited screening, informationwill be
embedded in the barcode of his or her
boarding pass. A
logo or thewords
TSAPREwill alsobe printed on the boarding
pass near the name. Eligible passengers should
proceed to the
lane.When the
TSA scans the barcode at designated
checkpoints, eligible passengerswill be
directed to an expedited screening lane. It’s
important to note thatwhile the
logo orwords TSAPREwill appear on all
qualifying boarding passes, not all airports
currently offer a
lane.

• Ensure the secure flight data (name, DOB,
gender, KTN andoptional redress number)
in all bookingprofiles and reservations
matches your Trusted Traveler profile.
• Enter theKTN/PASS ID/DoD ID number
intoyourMileagePlus profile at
united.com/tsaprescreening at least 72
hours before departure. If your next flight
departswithin72hours of the update,
also add theKTN/PASS ID/DoD IDnumber
directly to the reservation on united.com
by going to Change/view existing
reservations—Traveler information—
Known traveler number. Enter the
number and SUBMIT to save it to the
reservation.
• Reprint your boardingpass if you enter or
