Steps to Apply for or Switch Home Care Providers in Victoria

Part 1: Applying for a Home Care Package (New Clients)
Step 1: Call My Aged Care
Contact the government’s My Aged Care service on 1800 200 422 or visit myagedcare.gov.au to begin the registration process.
Step 2: Have an ACAT Assessment
An Aged Care Assessment Team (ACAT) will visit you at home to determine your eligibility and level of support—Level 1 to 4, depending on your care needs.
Step 3: Receive Your Approval Letter
After the assessment, you’ll receive a letter confirming your package level and that you're on the national waitlist. Wait times can vary between 1–12 months.
Step 4: Research and Choose a Provider

If your current provider isn’t meeting your needs, it’s okay to switch. Here's how:
Step 1: Notify Your Current Provider
Let them know you're ending your services. You'll need to give a minimum of 14 days’ notice.
Step 2: Call My Aged Care for a Referral Code
Request a new referral code to give to your next provider.
Step 3: Choose Your New Provider
Contact EnableU or your new chosen provider and provide your referral code to start the transition.
Step 4: Begin With New Services
Once your old agreement ends, your new services can begin right away—usually with no interruption in your care.
