Instructions: Please complete this form and the form for the specific
design/tee type selected showing the sizes and quantities.
Within 3 days of us receiving your order, you will be supplied with digital mockups of the designs and shirt
colors/styles selected for your order. You must approve these mock ups.
A 50% Deposit is required at the time of the order. We will email the listed contact an invoice after we
receive your order. That invoice is your confirmation that your order has been placed. We will contact
you to confirm the dates of your order. The remainder of the balance must be paid prior to the order
shipping. Once your order has been confirmed, we cannot cancel it. Shipping will be added at the end
after we know the total weight. Shirts in group orders ship together to one address. (Cost is usually $0.65-
$0.75 per tee for U.S. zip codes).
We do our best to ensure that every t-shirt purchased meets our high-
quality standards for construction and printing. We generally cannot
reprint or replace defective shirts from group orders. Any shirts found to
be defective within 30 days of receipt can be returned for a full refund,
including taxes and proportionate shipping charges or you may opt to
select a comparable shirt from our inventory, if available.
Orders take approximately 4 weeks to receive (U.S. orders) from the date
the order is placed until you receive it in your hands. (Approximately 2
weeks for printing, 1 week for packaging, and 1 week for shipping).

I understand that unauthorized use of any designs that infringe upon the
intellectual property rights of others without express permission from the
owner may violate certain laws and regulations. I agree to indemnify and
hold Reeve's Tees, LLC and its officers, directors, employees, affiliates,
agents, licensors, and business partners harmless from and against any and
all costs, damages, liabilities, and expenses (including attorneys’ fees and
costs of defense) Reeve's Tees, LLC or any other indemnified party suffers in
relation to, arising from, or for the purpose of avoiding, any professional
negligence or any claim or demand from a third-party that my use of their
intellectual property violates any applicable law or regulation, or infringes
or misappropriates the copyrights, trademark rights or other rights of any
third-party.
