Completing Applications and Examination Eligibility Verification (EEV) Report* Before applying for certification, please ensure that your students have the correct name of your program so that they choose the right name from the drop-down menu on the application. This will ensure that your students appear on the correct EEV Report. Before signing the EEV, be sure to check the program category as well (e.g., Medical Laboratory Scientist vs. Medical Laboratory Technician). The field on the Examination Eligibility Verification (EEV) Report titled "Program End Date" reflects the actual date of program completion as indicated by the student when they complete the online application form. Please ensure that your students are informed of the correct program end date to enter on the online application. This will affect your 2017 PPRs if you choose "Program Date."
