This is really really really obvious but everyone still finds it hard to do. What? Communicating, of course!
Even with all these great new technologies we still get frustrated when someone didn't say he was going to do that and then you tried doing it. Or she didn't announce that they've made this decision and so you told everybody in your team something else. The list goes on and on...
There is no easy answer, you've just got to think about it really hard. Keep it in your mind, put notes up reminding you the whole time. Tell people what you're planning, why you make decisions and when you're going to do things. It doesn't need to be formalised in meetings, to paraphrase the commercial, "Just communicate it!"
