To claim a Sure Start Maternity Grant you can either:

• print the form, fill it in with a pen and sign and date it
• fill in the form on screen and print it, then sign and date it with a pen

   Revised Sure Start Maternity Grant form SF100 (English and Welsh). Time allowed to claim a Sure Start Maternity Grant increased from 3 months to 6 months - pages 2 and 11.
