This prison service instruction (PSI) contains detailed information and general guidance on how to identify, manage and support prisoners who are at risk of harm or death in custody or who may be a risk to other people within the prison.

It also covers:

  • support for anyone who is affected by incidents of harm or death in the prison
  • investigating incidents of harm or deaths
  • best practice for continually improving safer custody services

This is a current PSI. It came into effect on 1 April 2012. It was last updated on 27 January 2020.
