   Published an updated version of form CRU1 Register a claim for compensation.

2. 5 April 2019

   Replaced all forms except CRU1503 with new versions. Removed order form for CRU1 form as this service no longer exists.

   Replaced CRU1503 form: NHS charges, contributory negligence - request liability review, with revised version.

   Replaced CRU 1 form with 2 page printable version.

   Replaced CRU forms with amended versions.

   Replaced CRU forms with updated versions (unlocked to allow completion in word processing software).
