The final destination of a journey is not, after all, the last item on the agenda, but rather some understanding, however simple or provisional, of what one has seen.
In these modern times, with the plethora of blood-sugar-related diseases, we need tools like GI and GL to help us understand ways to control blood sugar.
The self-testing, graphic approach to food testing developed in the balance of the newsletter is a less scientific but a more dynamic way to explore postprandial (post-meal) blood glucose levels (BGLs). GI measures the blood glucose impact of foods eaten in isolation, yet we rarely consume foods this way. GI readings vary with the individual—blood sugar and insulin reactions are more extreme for diabetics, for example (See Charts 2A and 2B). GIs are calculated in the science lab as the day’s first meal after a 12-hour fast and using a fixed serving that includes 50 grams of carbohydrate.  Most of our daily calories, however, are consumed in combination and throughout the day, when our blood sugar is affected by other foods that we have eaten earlier, as well as by our level of activity. Of the following numbered charts, the first three are based upon scientific research journal articles (Charts 1, 2A, 2B), while the last four (Charts 3-6) are constructed from my own self-testing of foods4 using a simple blood glucose monitor.

Ramekins filled with condiments like nuts and seeds (GI=0).  Nuts and seeds provide healthy fats, fiber, vitamins, minerals, and antioxidants, while they slow digestion and curb blood sugar. Sourdough bread or whole-grain bread with whole kernels; butter from grass-fed cows and organic nut and seed butters such as tahini and pumpkin seed butter. A pitcher of water and glasses for all—sometimes we mistake hunger for what is in fact thirst.  You might flavor the water with a little lemon juice or other flavoring. Because 12-hour fasting, pre-meal blood sugar reading can vary, all data points at time zero prior to the first morning meal were indexed to zero in order to illustrate the change from a neutral starting point. I use the label “traditional” carbohydrates, just as we call unrefined fats, “traditional” fats. What is the difference between type 1 and type 2 diabetes (and what if I don't fit either type)?

Rebounding (the so-called Somogyi phenomenon) is commonly cited by diabetes health care practitioners as a cause of high morning blood glucose readings.
Gee, I could get writer’s cramp listing every possible cause for erratic blood glucose readings. It is important to be aware that A1C levels are measured in different units and on a different scale than is blood glucose; hence an A1C level of 7 is not the same as saying that your average blood glucose is 7. Typically, type 1 diabetes in children, adolescents or teenagers and require immediate institution of insulin. Typically, type 2 diabetes develops in middle-aged or older persons who are overweight and can – at least initially - be managed with lifestyle therapy and non-insulin medications. There are also a large number of adults who, based on their age (and, often, based on their body size) are thought to have type 2 diabetes, but who don’t get their blood glucose levels down despite usual therapy with lifestyle and non-insulin medications and, soon after diagnosis, require insulin therapy. One helpful way to distinguish type 1 from type 2 diabetes is to do a blood test for a special protein called a GAD antibody. As you can see, if you have type 2 diabetes then at the time you were diagnosed your pancreas was only making ½ of the insulin it was supposed to and, as time passes, it progressively loses more and more of its ability to make insulin. If you have type 2 diabetes and are on insulin therapy you still have type 2 diabetes, not type 1 diabetes. ACE inhibitors and ARBs (I list the various types below) are medications that were created years ago to help control high blood pressure, but medical research subsequently discovered that even if a person with diabetes does not have high blood pressure, taking an ACE inhibitor or ARB will still reduce the risk of having a heart attack or stroke if you are at high risk for these complications.
Statins are medicines that improve cholesterol levels in your blood and they do this very well indeed. You may be thinking you have adrenal fatigue but is there testing you can do for adrenal fatigue?
So if you really want to make sure and put your mind at ease you will find this article helpful. One way of knowing is by monitoring how you feel and some of the symptoms that are commonly experience by people with adrenal fatigue.
One of the most accurate ways to test cortisol levels is with a saliva test that is taken over different time periods throughout the day. Cortisol is what we want to be measuring because generally the effects of adrenal fatigue and dysfunction occur due to cortisol levels that are all out of whack. When you measure your salivary hormones it shows the tissue concentration of the hormones in your body. You can ask your doctor for the tests but don’t be surprised if your medical practitioner does not recognise your symptoms and therefore not test you.


So you can order a home test, do the saliva test yourself, then send it into the lab for analysis. For my US readers, Canary Club has a home test you can purchase for $119 that gives you full instructions on when to take your saliva over a 24 hour period. This is where it can sometimes get confusing because different labs have different ranges for cortisol levels, this is known as the reference range. As you can see from the above image, the ranges are slightly different to the ones I provided above but what the image demonstrates is the normal cortisol curve. As you can see it is higher in the morning and lower at night, but when moving through the continuum to adrenal fatigue, the normal curve can be affected. These curve changes are the indicators that can help you determine if you have adrenal health issues that need to be addressed.
So in the meantime if you suspect adrenal fatigue, if your energy is flat, then start putting some of the dietary practices in place. It’s amazing how much better you can feel by simply changing a few things, such as giving up caffeine and sugar, or at least the majority of it. The second factor—the postwar shift from traditional to refined carbohydrates—is largely due to the growing role of the commercial food industry and processed, convenience foods.  Convenience foods must have a long shelf-life, so food companies rely upon refined flours and oils, which do not go rancid.
Visual pictures of postprandial blood sugar behavior, while less scientific than GI measurements, are nevertheless powerful learning tools, providing a real flavor for how our body reacts when we eat different kinds of foods. This chart illustrates the second meal effect– that what we eat at one meal affects postprandial blood sugar behavior at the next. What we do to our children when we give them a sugary cereal or a Pop-tart for breakfast extends beyond this first meal to affect their blood sugar, hunger, concentration, and desire to overeat throughout the rest of the day.
One of the best herbs and spices to moderate blood sugar.  It can be sprinkled on hot cereals and desserts such as puddings, custards, and stewed fruits. The dawn phenomenon is the rise in blood glucose levels in the dawn (that is, the morning) due to excessive release of glucose from the liver into the blood.
This individual, like so very many others living with diabetes who have high blood glucose levels first thing in the morning, blamed themselves and attributed their elevated morning blood glucose to having overeaten or snacked the night before. Not so! This is particularly likely to be the case if you are taking NPH insulin at supper or Levemir insulin (only) in the morning. If you are rebounding it means that you had a very low blood glucose overnight, slept through it, and your liver poured out so much glucose into your blood to bring your blood glucose back up that it ended up making too much and overshot the mark and as a result your blood glucose was high when you awakened. The idea is that a certain amount of carbohydrates is likely to raise your blood sugar level by a predictable amount and thus, you can take an amount of insulin proportional to your carb intake to prevent this rise in blood glucose from occurring. Indeed, there are many middle-aged people who develop type 1 diabetes and there are increasing numbers of children who develop type 2 diabetes. Such people often have a form of type 1 diabetes called LADA (Latent Autoimmune Diabetes in Adults). This antibody is almost always present if a person has type 1 diabetes and is almost always absent if someone has type 2 diabetes. Well, often it doesn’t, but there is at least one situation where it does; that is, the instance where a person with LADA is unsuccessfully treated with oral medicine after oral medicine in a fruitless attempt to control their blood glucose levels when what they really need is insulin! The great thing about measuring saliva in this way is that it shows the active component of the hormone in your body, whereas a blood test would show total hormone levels but not necessarily the active component. It is unfortunate that many doctors only recognise the very end points of adrenal dysfunction, cushings and addisons disease. And if you are the type of person who might get motivated more from the results of a test, then perhaps this option is a good one for you. But there are days when I don’t maintain regular meal times – and when you eat is as important as what you eat. Write down every little thing you do to maintain a healthy diet and every bit of exercise you do. Eating a little at a time throughout the day won’t help unless you’ve got a professional diet plan that keeps you within your daily calorie intake limit.


Though you won’t see results in a few days, you need to consistent for a few months and also need to take care of what you eat. The usual best way to deal with the dawn phenomenon is to take a dose of NPH, Lantus, or Levemir insulin at bedtime. There is always a reason for erratic blood glucose levels and if you’re having this problem, you and your health care providers need to play Sherlock Holmes and find out what is the cause! But if stress is part and parcel of your life (as it is of so many lives) your blood glucose levels can still be well controlled so long as your treatment program is tailored appropriately to your needs. Therefore, even if you aren't yet at the target A1C (and indeed even if you never reach target) it is important that you know that any reduction in your blood glucose (and, hence, your A1C) will substantially reduce your risk of diabetes damaging your body. But, if you don't keep a log book then you are not going to be able to recognize trends and patterns in your readings.
Carbohydrate counting is essential if you are using an insulin pump and is often of major value if you are taking injections of rapid-acting (Humalog, NovoRapid, Apidra) before meals; especially if you have type 1 diabetes. I truly believe that good food is the key to a happy, healthy life and I'm on a mission to inspire you to get back inside your kitchen, eat real food, and as a result, improve your health dramatically. Buying a pack of something from your local store may seem convenient, but packaged stuff is full of sugar, fat or salt and will certainly not speed up your weight loss.
This is why diabetes and obesity often go hand-in-hand (90% of diabetics are either overweight or obese). David Ludwig regarding high-glycemic foods and overeating, cited in the Recommended Reading section at the conclusion of this newsletter.
If you are already taking one of these insulins at bedtime then it may be that your dose needs to be adjusted.
Except that recent medical studies suggest this doesn’t actually happen and that rebounding doesn’t even exist! If you are able, try to have a straight shift (regardless whether this is days, nights, or something else). So if your A1C was 10 and you get it down to 9 pat yourself on the back; you've just reduced your risk of eye and kidney damage by almost 40%! Speak to your diabetes educators (in particular, your dietitian) to see if this would be a helpful technique for you. If you are working on losing weight, include a moderate exercise plan to help your body adjust and lose it.
To see progress in your healthy weight goal, you want to ensure you minimize your sugar, salt and fat intake.
My strong expectation is that within the next few years we will have conclusive evidence for the non-existence of rebounding.
If this is not possible, your blood glucose control can still be maintained but your insulin schedule may need to be overhauled. It reveals the patterns and provides the clues as to what changes to make to your therapy, be they dietary, insulin or otherwise. So here you'll find easy and practical info to help you eat well, and feel your best everyday.
When you walk regularly, the benefits are equal to other forms of exercise such as jogging.
My preferred strategy for my patients with type 1 diabetes who perform shift work is for them to use an insulin pump or, failing that, using Lantus (or Levemir) insulin once daily (given at the one time of day they are always certain to be awake) and a rapid-acting insulin (Apidra, Humalog, or NovoRapid) before meals (whenever those meals happen to be for a particular shift). A log book is not meant to simply be a historical record and it is most definitely not a report card. Some years ago, I also signed up with an aerobics class – it was a fun way of keeping fit.
