            You are being asked to voluntarily participate in a research study. I am doing this study to determine how
            information in course syllabi affect perceptions of the syllabus. If you choose to participate, you will be
            asked to read a sample syllabus and answer survey questions about it.

            If you agree to take part in this study, you will receive $0.50.
        </p>

        <p>
            We will not record your name or any information that shows your identity. You will not be signing this
            form.
        </p>
