Hello, my name is [name of enumerator]. I am working for BRAC University. We are conducting a research study in association with Stanford University and Queen’s Univerisity. We are inviting [name of male and female respondent] to participate in a research study on men and women’s health and life experiences. Your house has been chosen by chance (as in a lottery/raffle) to participate. We are requesting your permission before we start the survey with them. Before you decide whether or not to to let them participate in the study, I would like to explain the research briefly and what their participation in the project would entail. Please feel free to ask questions if there is anything that you do not understand. As part of the study we will do a survey involving food and sound that will help us understand how men and women make household-related decisions, such as what food to consume. They will participate in this part of the survey with a man/woman from a different household. This man or woman will never know who [name of male and female respondent is and neither will [name of male and female respondent learn who s/he is. The plate chooser in this survey will choose a plate for him/her self and the sound responder will use an annoying sound in response. First, we will do a lottery to determine whether they will be participating with a man or an woman from another household. Second, we will do another lottery to determine whether they will be the plate chooser or the sound responder. We will also tell them a story about a husband and a wife who lives in a fictional village just like this and ask their opinion about it. Finally, we will ask them questions about the household composition, employment, health, as well as their opinions about different household matters. In total, their participation will take approximately 2 hours. They will earn at least 100 BDT each as payment for their participation and any food they receive as part of the food and sound portion of the survey. We hope that their responses will help guide policies to improve the health and well-being of men, women, and children in rural areas in Bangladesh. Everything that they say to us is confidential and will only be shared confidentially, without their name, or the name of anyone else in your household, mentioned. Identifiers might be removed from identifiable private information and, after such removal, the information could be used for future research studies or distributed to another investigator for future research studies without additional informed consent from you or them. Finally, decision to participate or not participate will not have any effect on any household members in the future. There is no risk, inconvenience or benefits for you in the future if you decide to let them participate in the study. We cannot and do not guarantee or promise that you will receive any benefits from this study. Whether or not you consent to their survey is completely up to you. পরিশেষে আমরা তাদের কাছে পরিবারের গঠন, কর্মসংস্থান এবং স্বাস্থ্য এবং পরিবারের বিভিন্ন বিষয়ে তাদের মতামত জানতে চাব।
সর্বমোট তাদের সাক্ষাতকারটি সম্পন্ন করতে প্রায় দুই ঘণ্টা সময় লাগবে। তারা প্রত্যেকে সাক্ষাতকারটিতে অংশগ্রহণের জন্য কমপক্ষে ১০০ টাকা পাবেন এবং খাবার এবং শব্দের অংশ হিসেবে খাবার গুলো পাবেন। আমরা আশাবাদি যে, তাদের অংশগ্রহণ বাংলাদেশের গ্রামাঞ্চলে পুরুষদের, মহিলাদের এবং শিশুদের স্বাস্থ্য ও সমৃদ্ধি উন্নত করার নীতিমালা / গাইড প্রস্তুত করতে সহায়তা করবে।
আমরা আশা করি না যে আপনি যদি তাদের এই গবেষণায় অংশগ্রহণ করার ব্যাপারে সিদ্ধান্ত নেন তাহলে আপনার জন্য কোনো ঝুঁকি থাকবে।
তারা আমাদেরকে যা বলবে তা সবই গোপনীয় থাকবে এবং শুধুমাত্র গোপন ভাবে, তাদের নাম বা আপনার পরিবারের অন্য কারো নাম উল্লেখ না করে গবেষণার কাজে ব্যবহার করা হবে। শনাক্তকারী ব্যক্তিগত তথ্যাদি গুলো (পরিচয়) অপসারণের পরে বাকি তথ্যাদি ভবিষ্যতে অন্য গবেষণার জন্য ব্যবহার করা যেতে পারে। এছাড়াও ভবিষ্যতে বাকি তথ্যাদি পুনরায় আপনার বা তাদের অবহিত সম্মতি ছাড়াই অন্য তদন্তকারীর কাছে ভবিষ্যৎ গবেষণার জন্য হস্তান্তর করা যেতে পারে।
