You are invited to participate in the research study "Supply Chain Game", in collaboration with TU/e and TiU. The objective of this research project is to investigate planning decisions in supply chains. Please read this consent form and ask any questions you may have before agreeing to participate in the study. Participation is voluntary, and you can change your mind at any time.

Task and Duration: If you agree to participate in this study, we will ask you to answer a few simple questions and play a supply chain game. We will also ask you some demographic questions such as gender, age, education level and field of study. The entire study is not expected to take more than 90 minutes.

Risks and Benefits of Being in the Study: The researcher does not anticipate risks beyond those encountered in normal day-to-day life. If for any reason you feel that you do not want to complete the study, just close your browser and you will be excused from further participating in the study. There are no consequences for you other than that you will not receive payment for participating in the study.

The benefits to participation are helping advance the field of supply chain management.

Voluntary Participation: Your decision to participate does not affect your current or future relations with the researcher. You have the right, in principle, to request (1) access to and rectification of, (2) erasure of, (3) restriction of or object to the processing of personal data. Incentives for participation: You receive a monetary compensation for participating in this study based on your performance in the experiment (min 5 euros, 20 euros on average).

Responses: The records of this study will be kept private and anonymized. In any sort of report we might publish, we will not include any information that will make it possible to identify a subject. Research records will be stored securely and only researchers will have access to the records. All anonymized data will be stored for ten years, will never be used for commercial purposes, and only might be used for other scientific research.

Finally, this research project has been assessed and approved by the ethical commission of Eindhoven University of Technology.
