<LI> I have read the Information Sheet regarding this project, and I agree to participate in the project named above. A written information sheet has been given to me to keep.</LI>
<LI>	I acknowledge that: </LI>
    <ol type ='a'>
<li>	I am free to withdraw from this research project at any time during the experiment, however if I decide to
    withdraw, I will forego my earnings from the experiment except for the $10 participation fee. </li>
        <li>I will be asked to undertake several computerized tasks, which involve making
            a series of economic decisions and answering some descriptive questions about myself.
            The experiment will take approximately 1.5 hours to complete. </li>
        <li>My payment will depend on my own decisions, the decisions of others in the lab with me and chance.
            I will receive $37.5 on average which included a participation payment of $10. I will be paid in cash and/or
            transfer payment at the end of the session.  </li>
<li> Any data collected during the experiment will be recorded only by an anonymous subject number, and all participants shall remain anonymous. </li>
        <li>The data collected will be stored on the password-protected computers of the researchers and on the
            UQ Research Data Manager (UQ RDM), will be used in the above-mentioned study and may be used in future studies.
            The data will be used only for research purposes.</li>
<li> For the purposes of payment, I need to provide my name and my signature on the receipt form. This data is collected
    solely for the accounting purposes and will not be connected in any way to the decisions I make in the experiment.
    I understand that my name will not be associated in any way with the research findings, and that my contribution will be completely confidential.</li>
        <li>This study adheres to the Guidelines of the ethical review process of The University of Queensland and
            the National Statement on Ethical Conduct in Human Research. I can discuss my participation in this study
            with the project staff (Pimwilai Kijjanapanich is contactable on 3365 6570 and via p.kijjanapanich@uq.edu.au),
            and I can contact the Ethics Coordinator on 3365 3924.</li>
