Study Title: Decision making experiment

Principal Investigators: Bohan Ye

This is a consent form for research participation.   It contains important information about this study and what to expect if you decide to participate.  Please consider the information carefully. Feel free to ask questions before making your decision whether or not to participate.

Why is this study being done? The purposes of this study are to gain understanding how individuals make decisions in a simple task setting.

What will happen if I take part in this study? You will be asked to do a simple performance task in which you will go through a series of computer screens and make some decisions. This task is entirely on the computer. Detailed instructions about this task will be provided in the experiment.

How long will I be in the study? The experiment is expected to last 45-60 minutes.

How many people will take part in this study? This study will be conducted with approximately 800 University of Arizona students.

Can I stop being in the study? Participation is voluntary. You may refuse to participate. If you do take part in the study, you may withdraw at any time without incurring any ill will.  No matter your decision, you may keep your $5 participation fee, but you will not earn any additional compensation from this study. Your decision will not affect your future relationship with The University of Arizona in any way.

What risks, side effects or discomforts can I expect from being in the study? There are no risks for this experiment beyond those from any other typical activity in a University of Arizona classroom or computer lab.

What benefits can I expect from being in the study? You will be paid your experimental earnings today based on the decisions made by yourself and other participants in this session.

What other choices do I have if I do not take part in the study?

If you do not take part in this study, you will receive your show-up fee, but must leave the lab.

Will my study-related information be kept confidential? The confidentiality of any personal information will be protected to the extent allowed by law. Only the researchers will have access to the data from this experiment and your name will not be attached to the decisions you make or information you provide.

Who can answer my questions about the study?

We have described the general nature of what you will be asked to do but the full intent of the study will not be explained to you until after your participation. At that time, we will give you more information about the study and give you an opportunity to ask questions.
