 1.	I confirm that I have read the participant information sheet dated <b>27 JUNE 2023 (version 1.2)</b> for this study and that I am eligible to take part.

 2.	I understand that some of the questions may be perceived as sensitive and that if I need to, I can access the sources of support included in the participant information sheet and end of study information.

3.	I understand that my participation is voluntary. I am free to withhold personal information and can stop the survey at any time without giving any reason, without my medical care or legal rights being affected.

4.	I understand that the information collected about me might be used to support other research in the future, and my anonymous information may be shared with other researchers.
</p>

5.	I understand I may quit the survey at any time but any data I have provided may still be used.
</p>
