 <b> This is a consent form for research participation.</b>
    It contains important information about this study and what to expect if you decide to participate.<br>
<b>Your participation is voluntary.</b>Please consider the information carefully.
    If you decide to participate, you will be asked to consent to this form.
    <br>
    <b>Procedures/Tasks:</b> You will get 30 résumés. Each résumé includes 1) Cognitive skill test score and 2) Social skill test score, as well as the candidate's name or number.<br>
    Your job is what will each candidate's performance for other sets of social skill tasks and cognitive tasks, and a task that requires both skills.<br>
    <b>Purpose:</b> The object of this session is to evaluate résumés, as the second part of the study.<br>
    You will not be fully informed about the nature of the study before your participation and completion of the tasks.
     <br>

    <b>Duration:</b>Tasks will last approximately 15 minutes - 20 minutes. <br>
You may leave the study at any time.  If you decide to stop participating in the study,
    there will be no penalty to you,
    and you will not lose any benefits to which you are otherwise entitled.<br>
    <b>Risks and Benefits: </b> There are no foreseeable risks to you from your participation in this experiment.
    The primary benefit of the study will be to advance our understanding of decision-making processes.<br>

 <b> Confidentiality:</b><br>
     Efforts will be made to keep your study-related information confidential.
    However, there may be circumstances where this information must be released.
    For example, personal information regarding your participation in this study may be disclosed if required by state law.
    We will work to make sure that no one sees your survey responses without approval.
    But, because we are using the Internet, there is a chance that someone could access your online responses without permission.
    In some cases, this information could be used to identify you.
Also, your records may be reviewed by the following groups (as applicable to the research):
<ul>
<li> Office for Human Research Protections or other federal, state, or international regulatory agencies;</li>
<li> The Ohio State University Institutional Review Board or Office of Responsible Research Practices;</li>
    <li>The sponsor, if any, or agency (including the Food and Drug Administration for FDA-regulated research) supporting the study.</li>
</ul>
Data from the experiment will be shared with interested researchers, and when the research is published, on the journal’s web site.
There are no names accompanying any of this shared data. <br>
<b>Incentives: </b> There is a good deal of variation in earnings between individuals in any given experimental session
so that the amount you earn will depend on the decisions made by you and on chance (random) factors (on average $2.5).
    Also, you will get $0.5 participation fee if you complete the tasks.
If you choose to end participation before the session is over, you will not receive any of the earnings.
Payments will be made in Prolific payment form at the end of the session.  There are no costs to participating in the study.<br>
<b>Participant Rights:</b> <br>
You may refuse to participate in this study without penalty or loss of benefits to which you are otherwise entitled.
 <br>

If you choose to participate in the study, you may discontinue participation at any time without penalty or loss of benefits.
By consent this form, you do not give up any personal legal rights you may have as a participant in this study.<br>

This study has been determined Exempt from IRB review. <br>
