Amend SB 1188 (house committee report) as follows:

(1)  On page 4, line 16, between "SEX" and the underlined period, insert "AND GENDER IDENTITY".

(2)  On page 5, strike lines 6 through 8, and substitute the following:

(b)  An electronic health record must include spaces for recording other information related to an individual\'s gender identity.
