This article originally appeared in the July 2011 issue of The American Journal of Medicine.
To the editor:Cacoub et al (1) provided us a useful list of 44 drugs leading to a Reaction with Eosinophilia and Systemic Symptom (DRESS) and analyzed the potential mechanisms such as hypersensitivity with genetic susceptibility or HHV-6 reactivation.
Although 50 drugs can induce DRESS, the main culprit drugs are carbamazepine and allopurinol.

Diagnosing DRESS is challenging due to the diversity of cutaneous eruption and organs involved. Approval of Incivek (telaprevir), a direct acting antiviral drug (DAA) to treat hepatitis C (HCV). We used the RegiSCAR scoring system that grades DRESS cases as “no,” “possible,” “probable,” or “definite” to classify cases reported in the literature. You can read the accompanying article in the August 2016 issue of the American Journal of Medicine. A total of 44 drugs were associated with the 172 cases reported between January 1997 and May 2009 in PubMed and MEDLINE.

This better knowledge of DRESS may contribute to improve the diagnosis and management of this syndrome in clinical practice.
