NB: The hypothesis of the TAME study is based upon promising research on metformin conducted by the scientific community. If indeed metformin is a drug that can target the aging process, its administration should be associated with less age-related disease in general, rather than the decreased incidence of a single age-related disease. Finally, a large population-based study evaluated the effect of metformin on five-year survival.
NB: The main reason to embark on this study is to convince the Food and Drug Administration (FDA) to approve aging as an indication, and thus a target for future and even better medications. Without such a determination, the progress the field has made will not be realized because pharmaceutical companies will not develop drugs that have no indication, which is required for reimbursement by insurance companies. Diabetes remains the seventh leading cause of death in the United States, with over a million new incident cases being reported each year, according to the American Diabetes Association (ADA). Not only is diabetes detrimental to one’s health, but it is also a costly disease to treat. The Department of Medicine, Division of Endocrinology, Diabetes and Hypertension at UCLA provides services for a full range of endocrine problems. In addition to care, the center incorporates key clinical research to investigate the pathophysiology, prevention and cure of diabetes and its complications.
When it comes to diabetes and obesity, leading experts at Northwestern have embarked on a campaign to create a cross-disciplinary diabetes center to continue research, education and clinical care. At Yale-New Haven Hospital, the Diabetes Center was founded in 1994 to serve as a regional resource for diabetes diagnosis, education and treatment.
The multidisciplinary team guides each patient through diabetes management using the latest technologies available. The Division of Endocrinology, Diabetes, and Metabolism at New York-Presbyterian takes an active role in using innovative treatments and educating the public on how to prevent the lifelong disease. The team is composed of diabetologists, a certified diabetes educator, a registered dietician, and a podiatrist. The Diabetes Treatment Center at UCSF Medical Center provides instruction and information about diabetes management, emphasizing the importance of individualized care and patient empowerment by teaching people how to best manage their condition. The Massachusetts General Hospital Diabetes Clinical Center is one of the oldest outpatient centers in the country dedicated to the comprehensive treatment of persons with diabetes, pre-diabetes, and associated disorders.
Cleveland Clinic’s Diabetes Center provides patients access to a multidisciplinary team, including endocrinologists, diabetic educators, dieticians and a nurse practitioner. The Diabetes Center offers both individual and group diabetes education classes year round for patients and their families.
Mayo Clinic is the first and largest integrated, not-for-profit group practice in the world. Additionally, the Mayo Clinic offers a comprehensive diabetes self-management education program for nearly 4,000 patients and their families each year. This chronic illness has serious complication up to and including death.  To create awareness about this silent epidemic November has been designated American Diabetes Month.

Aboriginal and Torres Strait Islander visitors to the Diabetes Queensland website are advised that this website may contain images and voices of deceased persons. This notion led investigators to further study whether anti-aging effects can be demonstrated in the type 2 diabetes population. In fact, metformin’s potential protective effect against cancer has been gaining much attention, with over 100 ongoing studies registered on the Clinical Trials.gov website.
The design, population selected and data obtained, will not be detailed here, but is a base for our analysis. It is sponsored by the American Federation for Aging Research (AFAR), and I will serve as the PI.
Indeed, metformin is a cheap generic drug that has already shown to have protective effects in diabetic patients, so it seems to be a good tool.
Chronic metformin associated cardioprotection against infarction: not just a glucose lowering phenomenon.
Comparative effectiveness of sulfonylurea and metformin monotherapy on cardiovascular events in type 2 diabetes mellitus: a cohort study.
Effect of metformin on cardiovascular events and mortality: a meta-analysis of randomized clinical trials. Effects of metformin versus glipizide on cardiovascular outcomes in patients with type 2 diabetes and coronary artery disease.
Reduced cardiovascular morbidity and mortality associated with metformin use in subjects with Type 2 diabetes. The total estimated cost of diagnosed diabetes in 2012 was $245 billion, according to the ADA, including $176 billion in direct medical costs and $69 billion in reduced productivity. The UCLA Diabetes Program provides both primary and consultative diabetes care to referred patients. A diabetes center, now in its seventh year of service, offers a multidisciplinary team approach, with a comprehensive case management and development of individualized treatment plans.
They are part of the UCSF Diabetes Center which is one of only 11 Diabetes Endocrinology Research Centers in the country, named by the National Institute of Diabetes, Digestive and Kidney Diseases. The Johns Hopkins Comprehensive Diabetes Center offers diagnosis, assessment, education, management, and multidisciplinary care.
Over the course of a five-year period, the Diabetes Institute will work to improve collaboration, philanthropy and faculty surrounding the treatment of diabetes mellitus at Johns Hopkins. With a closely integrated treatment center and clinical research center, the center’s ultimate goal is to cure diabetes. Chelsea HealthCare Center and Revere HealthCare Center allow patients with diabetes to have their eyes screened at the same place they receive their primary care.
The goal of the Diabetes Center is to encourage patients to receive early specialty care and education – getting them on the right track with their diabetes management before returning them to their primary care physicians for ongoing management. Another convenience offered is retinal eye screenings on-site to provide screening for diabetic eye conditions. The Diabetes Control and Complications Trial showed that tight blood sugar control prevents or slows the progressive complication of diabetes that affect the eyes, kidneys, and nerves. News & World Report and Magnet designation by the American Nurses Credentialing Center. At this rate one of every three children born today will become diabetic in their lifetime. Notably, in the United Kingdom Prospective Diabetes Study (UKPDS), metformin, compared with other anti-diabetes drugs, demonstrated a decreased risk of cardiovascular disease (7). Records of 78,000 diabetic patients treated with metformin were compared with records of 78,000 non-diabetic subjects and matched for relevant characteristics, such as age, smoking, cancer history, etc. There is a wide range of involvement of gerontologists and intervention geriatricians in an executive committee and consensus committee as well as other investigators. We are confident that once FDA becomes open to aging as an indication, more and better drugs will be rapidly developed.

If started early in life, metformin treatment increases life span and postpones tumors in female SHR mice.
New users of metformin are at low risk of incident cancer: a cohort study among people with type 2 diabetes. Type 2 diabetes increases and metformin reduces total, colorectal, liver and pancreatic cancer incidences in Taiwanese: a representative population prospective cohort study of 800,000 individuals. Inhibition of lung tumorigenesis by metformin is associated with decreased plasma igf-I and diminished receptor tyrosine kinase signaling.
Effect of metformin on IGF-1 and IGFBP-1 levels in obese patients with polycystic ovary syndrome. Caveolin-1 is essential for metformin inhibitory effect on IGF1 action in non-small-cell lung cancer cells.
Metformin decreases IGF1-induced cell proliferation and protein synthesis through AMP-activated protein kinase in cultured bovine granulosa cells. Metformin-mediated growth inhibition involves suppression of the IGF-I receptor signalling pathway in human pancreatic cancer cells. A comparison of mortality in people initiated with metformin or sulphonylurea monotherapy and matched, non-diabetic controls.
As part of the national designation, the centers receive federal funding for diabetes research.
By integrating diet, oral medication or insulin, and patient education, the Diabetes Center works to achieve the best possible outcomes, helping people with diabetes to live long and healthy lives. Another 57 million Americans are pre-diabetic and are at risk of developing Type 2 Diabetes.  That is why the American Diabetes Association’s has developed the “Stop Diabetes” campaign.
This has been suggested in other studies and meta-analyses and remains an active area of research (8-12).
Similar comparison was performed between 12,000 diabetic patients taking sulphonylurea (SU) drugs and 12,000 matched non-diabetic individuals. We believe that there is a great benefit for healthy lifespan, not only to the individual, but also for the society in the form of cost savings, which is often referred to as the longevity dividend. The difference today is that by creating collaborations, opportunities, and friendships with many others we have more than a village to succeed. The campaign’s goal is to rally millions of Americans in the fight against diabetes, as well as proactively guard their health to prevent it. Metformin also extends the lifespan of nematodes (5), suggesting an evolutionarily conserved mechanism. Not unexpectedly, SU-treated diabetic patients had ~40 percent greater mortality than their non-diabetic control group.
Jim Kirkland among others.  What do you enjoy most about working with other experts to find answers about why our bodies age?
My confidence in our success is because the partnership with great scientists is like the egg and bacon analogy. A recent high-impact study demonstrated that metformin reduces oxidative stress and inflammation and extends both lifespan and healthspan in a mouse model (6). However, mortality in metformin-treated diabetic patients was similar to the matched non-diabetic controls. In fact, among patients in their 70s, mortality was reduced by ~15 percent in metformin-treated diabetic patients compared with non-diabetic controls. These observations suggest that the protective action of metformin may extend beyond effects on specific age-related diseases and provide compelling evidence to support the design and conduct of studies to directly test whether human aging, and its diseases, can be effectively delayed. Kirkland, who also have the leadership abilities, will contribute to bringing this research to fruition.
