So far we have not seen too much resistance to the tetracycline class of antibiotics used today, but they too will be on their way out if we do not change our prescribing patterns. An oldie but goodie is benzoyl peroxide in a relatively low strength (2.5 percent, compared to the 8-10 percent that you get at the drugstore). It is possible to simultaneously provide good therapeutic options while decreasing the risk of antibiotic resistance through the judicious use of these agents. Tetracycline shouldn’t be taken with food, especially milk, which can negatively impact absorption. If systemic therapy is required, it should generally be tapered and a topical alternative substituted as soon as is feasible. However, the safety profile of topical retinoids differs from their systemic counterparts; though teratogenicity is a theoretical concern and use in pregnant females not recommended, the occurrence of fetal risk hasn’t been well documented. When possible, benzoyl peroxide should be incorporated into therapeutic regimens to decrease the emergence of antibiotic resistance. And research has shown that the desired anti-inflammatory effects can be achieved at sub-antibacterial dosing. Pathogenesis of Acne and Increased Antibiotic ResistanceThe pathogenesis of acne can be thought of as a cascade of events including sebum overproduction, follicular hyperkeratosis, increased proliferation of intrafollicular P.
Because such therapy doesn’t address the primary pathogenic processes or lead to “cure,” courses are often prolonged and can select out for resistant gram negative organisms or the development of substantial P.
As a result, taking tetracycline on an empty stomach can negatively impact compliance, whereas doxycycline and minocycline can be taken with food.
Most experts believe systemic therapy should be discontinued or tapered after 3 to 6 months if possible. This means that the needed dose is so low that it can't kill good bacteria or challenge pathogenic bacteria to become resistant. Dr Brigid Lee, of Christchurch's skin rejuvenation clinic, said acne was often the first sign of pubertal maturation.
Unfortunately, I am not able to accommodate most of those requests due to my schedule and the nature of my job. This procedure is fractional meaning that it is only exposing a part of the skin at a time to the laser. This internship program allows pre-pa students to work side-by-side with myself, other PAs and Physicians to experience health care and see what we do day to day.
