Cancer-related fatigue is a common side effect during cancer treatment, and research demonstrates that it is a troubling, lingering side effect for many long-term survivors.
Fatigue that lingers long into survivorship affects quality of life, mood, and work productivity.[4,5] A patient’s fatigue level at time of diagnosis may increase risk for fatigue problems after completion of treatment.
Hypokinesia has been examined in exercise studies that consistently demonstrate that regular, moderate exercise improves aerobic capacity and muscle strength and decreases fatigue. The National Comprehensive Cancer Network (NCCN) 2009 Practice Guidelines for Cancer-Related Fatigue include a treatment and intervention algorithm for long-term survivors.[11] After ruling out treatable contributing factors such as pain, emotional disorders, anemia, sleep disturbances, and medication side effects, the guidelines specify strategies for fatigue management. Women recovering from breast cancer often experience troubled sleep as result of their treatments – which can lead to increased fatigue and inflammation. For women recovering from breast cancer, exercise is one of the best ways to lower fatigue and inflammation. Researchers chose to try yoga with breast cancer survivors, because it can be used with all levels of fitness and can be adapted for women with physical limitations. Over a period of six years, researchers at Ohio State University studied the effects of yoga on inflammation, mood and fatigue levels in breast cancer survivors who had completed cancer treatment within the past three years. Furthermore, compared to traditional treatments for fatigue, inflammation and mood, yoga doesn’t have dangerous side effects.

The end result is that many cancer survivors end up trying to drag themselves through their days. She and her colleagues, including her husband and research collaborator, Ronald Glaser, recruited 200 women aged 27 to 76 who were new to yoga and had finished treatment for breast cancer within the last three years. At the six-month mark, the women practicing yoga reported about 60 percent less fatigue than the women on the waiting list, and their measures of inflammation were 13 percent to 20 percent lower.
Factors that contribute to severe fatigue in survivors include low levels of physical activity, depressed mood, impaired sleep and rest, and feelings of fatigue for as long as 1 year prior to diagnosis.[6] Some physiologic factors may also influence fatigue after completion of treatment. Basically, as one gets physically stronger, it is easier to perform desirable activities without becoming fatigued.
Though specific measures to assess fatigue in long-term survivors have not been developed, scales that measure fatigue during treatment appear to be psychometrically sound in the long-term survivor setting.
New research published in the Journal of Clinical Oncology found that yoga may be a safe way to reduce fatigue and improve overall health in breast cancer survivors. For survivors especially, this emotional support and understanding of their changing bodies can be helpful, according to Kiecolt-Glaser.
27, 2014 (HealthDay News) — Yoga may help breast cancer survivors beat the debilitating fatigue and sleep problems that often follow toxic treatments such as chemotherapy and radiation, a new study shows.

As many as 60 percent of cancer survivors say they have trouble sleeping, she noted, a rate that’s two to three times higher than their cancer-free peers. While all of these hypotheses are plausible, only the correction of anemia and hypokinesia (debilitation from physical disuse) have demonstrated a clear correlation with fatigue. The most efficient and relevant clinical measure of fatigue continues to be a 10-point scale (ie, 0-to-10 scale), which is easy to administer and is easily understood by patients.[7] Moderate to severe fatigue scores (4–10 on a 10-point scale) warrant further evaluation of underlying disease.
Breast cancer survivors have a 30 percent lower level of cardiorespiratory fitness compared to their sedentary counterparts who haven’t had treatment.
As clinicians, it is important to look for other treatable and reversible common causes of fatigue, including anemia, depression, anxiety, and hypothyroidism.
Women who had practiced yoga had 57 percent less fatigue and 13 to 20 percent less inflammation compared to the non-yoga group.
