Looking at the illustration, we can see that the outer ear-canal extends from the outside of the head along the auditory canal to the eardrum.
This congestion will result in an imbalance of air pressure between the middle ear and the external air pressure. When swallowing, we can hear the ears making a clicking sound because the throat muscles open up the Eustachian tube in the throat (see red arrows). On an airplane, barotrauma to the ear – also called aero-otitis or barotitis – can happen as the plane descends for landing. In more severe cases of barotrauma, the middle ear can fill with clear fluid as the body tries to equalize the pressure on both sides of the eardrum. The eardrum can rupture (break) in severe cases of ear barotrauma, causing bleeding or leaking of fluid from the ear. You can diagnose a mild case of ear barotrauma yourself, and you do not need to see a doctor. During a flight, make sure you are awake for the landing so you can "pop" your ears if necessary. If you experience dizziness that includes a feeling of spinning or falling (vertigo) and your symptoms occur right after flying or diving, you need to be evaluated by a doctor immediately because there is a small chance you may need emergency ear surgery.
TMD frequently occurs when the lower jaw is not in the proper position relative to the upper jaw. Swallowing is a very complex process and any interruption in the swallowing process can lead to dysphagia.
Oral preparation –The food is chewed, mixed with saliva and the tongue collects the prepared food or liquid for swallowing. Pharyngeal stage – The tongue pushes the food or liquid to the back of the mouth, triggering a swallowing reflex that pushes the food into the pharynx (throat). Many disorders of swallowing can be treated with medication, swallowing therapy or surgery. Ear infections – There are many types of ear infections caused by either bacteria, a virus or fungus.

Behind the eardrum there is a confined space (the middle ear) which is connected by a small tube - the Eustachian tube - to the back of the throat. This opens the passage for air to flow via the back of the throat into the vacuum of the middle ear, popping back the eardrum (this popping is often noticed at high altitude, for example in an aeroplane or in a car in the mountains). The only connection between your middle ear and the "outside world" is a thin canal called the Eustachian tube. This fluid is drawn out of blood vessels in the lining of the inner ear, and can only drain if the Eustachian tube is open. It is much more likely to happen to people who have colds, allergies or infections when they are flying. It occurs, rarely, in divers who hold their breath, when the diaphragm moves abruptly in a "gasping" effort. If you are uncertain about your symptoms or if your symptoms last a long time, a doctor can examine your middle ear with a lighted magnifying tool called an otoscope to see if the eardrum is pulled inward. If you have a cold, ear infection or allergy, you may want to reschedule airplane travel until you are better. In unusually persistent cases, an ear, nose and throat doctor may have to make a small incision in the eardrum to equalize the pressure and drain the fluid. If you have severe pain, bleeding or drainage of fluid from your ears, see a doctor within several days because you may have a ruptured eardrum. Frequently a team approach involving medical doctors, neurologists, ear, neck and throat specialists, chiropractors, and therapists is necessary to diagnose and treat patients with jaw joint, neck and spine dysfunctions. When the lower jaw goes back too far, this results in the protective disc becoming anteriorly displaced as per diagram.
Prolonged spasms of facial muscles can lead to chronic muscle pain and trigger points (knotted muscles), which can send pain to different areas of your body such as your neck, eyes, ears, shoulders or lower back which can make it difficult to diagnose. The external air pressure becomes greater than the pressure in the middle ear, and this pushes the eardrum inwards. The pressure change can create a differential between the outer and middle ear that pushes the eardrum inward.

In severe cases, it is possible for the pressure to create a leak between the deepest structures of the ear (the fluid-filled bony canals called the cochlea and semicircular canals) and the inner ear space. If you have a ruptured eardrum, you need to keep water out of your ear to prevent infection.
If you have mild ear pain or hearing difficulty that continues after flying or diving, you should see a doctor for help if your symptoms are slow to go away.
The eardrum will become stretched, which can cause earache, sometimes accompanied by a reduced or muffled sense of hearing. As a remedy for earache in children I use a combination of Apis and Levisticum in low potency, D2 and D3 respectively, 5 pillules three times daily.
This can unblock the Eustachian tube and alleviate the pressure imbalance in the middle ear. If a collection of fluid is not visible, your doctor may squeeze a puff of air into your ear canal.
A perforation of the eardrum that has not healed after two months may need to be repaired surgically. Camomile flowers can also help to settle the symptoms of blocked ears, a blocked nose, or sinusitis-like symptoms.
As you can see, the problem of earache in children is often not caused by the ear itself, but by congestion in the throat area! These bubbles are constantly moving into the middle ear, where they balance the ear's inner pressure.
