In cases where the middle ear function is blocked, damaged or occluded, the Baha system may be a better option as it bypasses the outer and middle ear altogether. Once the cochlea receives these sound vibrations, the organ ‘hears’ in the same manner as through air conduction; the sound is converted into neural signals and is transferred to the brain, allowing a Baha recipient to perceive sound. The Baha system utilizes Direct Bone Conduction, which allows the bone to transfer sound to a functioning cochlea – thereby bypassing the outer and middle ear. One of the benefits of choosing Baha is that you can ask your hearing care professional for a Baha demonstration, so you can hear the difference yourself.
Potential recipients can easily test the Baha so that they can experience the benefits before they have surgery.
The BAHA consists of three parts: 1) The titanium implanted portion (small screw in the bone in picture to the right), which is similar to a dental implant, 2) An external abutment (the cone-shaped part that connects with the screw), and 3) The sound processor (hearing device) that clips onto the abutment. The BAHA is used to for people with conductive and mixed loss hearing loss with chronic infections of the ear canal or the middle ear (area behind the ear drum), people with a very narrow ear canal or an absent ear canal due to a congenital ear malformation, and people who are deaf in one ear. People with chronic infections of the outer ear (ear canal) and the middle ear (area behind the ear drum) can benefit from BAHA. Congenital hearing losses caused by abnormalities or malformations of the ear canal or the middle ear can be treated with a BAHA. When the BAHA is placed on the deaf side, it picks up the sound and transmits it through the bone of the skull (bone conduction) to the inner ear on the hearing side. The Baha device has a new device called Baha Attract which involves placing a magnet under the skin and the outside device is held on by a magnet. Tinnitus is the phantom perception of sound that occurs within a person’s ears or head that originates from no external sound source. Although tinnitus is a common medical symptom and can be extremely annoying, it is also one of the most difficult medical conditions to treat. As mentioned earlier, silence in the environment can cause the individual to become more aware of the tinnitus. Currently, there exist hearing aids from most major manufacturers with specific features to help patients with tinnitus who also have hearing loss; these manufacturers include Unitron, Phonak, ReSound, Widex, Starkey, and Siemens. Today’s hearing aid technology uses digital sound processing to provide patients with optimal benefit.
The audiologists here at Ear, Nose, Throat, & Allergy Specialists work with all of the major hearing aid manufacturers including Unitron, Phonak, ReSound, Widex, Starkey, and Siemens. A bone-anchored hearing aid is an option for patients for whom a traditional hearing aid is not a viable option, such as for these patients who fit one of these categories. Patients with chronically draining ears, where the use of a hearing aid aggravates the infection, causes feedback, discomfort, and poor sound quality. Patients with a conductive hearing loss due to chronic mastoiditis or ossicular disease that cannot be surgically corrected. Patients with total hearing loss in one ear and a conductive hearing loss in the other ear. Patients with complete unilateral hearing loss due to acoustic neuroma, sudden deafness, or genetic unilateral hearing loss who are unable to successfully use CROS or BCROS amplification. We work with Cochlear BAHA, a device worn on a soft headband or a surgically implanted system that uses bone conduction, in which skull bone vibrations act as a pathway for sound to travel to the inner ear without involving the ear canal or the middle ear.
Prior to this surgery, a demonstration of and testing with the BAHA system is performed using the BAHA testband so that the patient can experience the results of this system prior to surgery. A cochlear implant is a surgically implanted device for people who do not experience appropriate benefit from traditional hearing aids. In many instances hearing aids may be used to alleviate or control distressing head noise (tinnitus). If a person has an impairment of the conductive type, he can expect maximum benefits from a hearing aid because discrimination ability is not greatly affected.
If the hearing impairment is of the sensori- neural or nerve type, the difficulty of adjusting satisfactorily to a hearing aid may be greatly increased. Whatever the type of hearing impairment, it is important to follow a planned program of “learning to use the hearing aid.” The ease or difficulty of hearing will vary depending on the loudness of background noises, the distance of the listener from the source of sounds, the clarity of speech or of music, and the lighting (which may enhance or may interfere with lip reading). Thirteen recommendations for learning to use a hearing aid for maximum benefit are described in the following paragraphs.
Your hearing aid amplifies noise as well as it amplifies music or speech and you may be disturbed temporarily to background noise. If your initial experience with the aid is unsatisfactory, remember that you are learning new habits, or rather, relearning old habits in a new setting.
Localisation of sound (the determination of the direction from which the sound comes) often presents a special problem to wearers of hearing aids. At first, hearing aid users tend to set the volume control at a level too low for efficient listening. Lipreading will help you in general communication with others; consider it an important supplement to the use of the hearing aid.

The device has many similar features to the BAHA device but is different in appearance and some functionalities. Many times the person suffering from tinnitus believes there is nothing that can be done to provide relief from the ringing.
Remember, it is the STRESS AND ANXIETY caused by the tinnitus that is the pathology, rather than the tinnitus itself.
A "sound-enriched environment" can be created to substitute the tinnitus with more pleasant environmental sounds such as music, a ceiling fan, or ocean waves or background noise provided by sound machines. The testband with the hearing device is placed around the wearer’s head, with the sound processor against the bone behind the ear. Hearing aid amplification may effectively mask out (keep the wearer from hearing) the tinnitus. One’s age, the severity of the hearing impairment, and the acceptance of the need for the aid may strongly influence one’s reaction to supplementing his own hearing with amplified sound. Most persons with this type of impairment become adjusted to using a hearing aid with very little difficultly.

Practice exercises will help to prepare the wearer to use his hearing aid in widely different situations. Let the way you feel be your guide.You can, over a period of several weeks, gradually lengthen the amount of time that you wear the aid. Normal hearing persons miss individual words or parts of sentences and unconsciously “fill in” with the thought expressed.
Normal hearing persons are aware of background noises too, but have learned to push them out of conscious awareness. Dining out may present special problems to the hearing aid user, so eat your first meals in public in a quiet restaurant with carpeted floors and draped windows, avoid noisy cafeterias. Although lipreading has many limitations, some words cannot be seen on the lip0s and some words cannot be distinguished from each other, lipreading combined with a hearing aid is often more satisfactory than is either alone.
If your hearing loss is not especially severe, you will probably be able, with a little practice, to use your hearing aid with the telephone.
In some hearing aids the induction coil is an integral part of the aid, and the cordless portion of the telephone is placed in contact with the case of the aid. Furthermore, with unilateral deafness the Baha can be used to conduct sound from the deaf side to the hearing cochlea on the opposite side. The BAHA has been in use since 1977 in Europe and has been FDA approved since 1996 for conductive and mixed hearing loss.
One-sided deafness patients with a BAHA device can hear people sitting on their deaf side without the need to turn their head for listening. Most people with tinnitus will complain that the tinnitus is worse when in quiet settings, such as at night when trying to sleep. However, if you have tinnitus, you should be evaluated to determine what the best options are for managing it. Research studies have shown that music is one of the most effective environmental noises helping people relax and cope with their tinnitus.
Binaural fitting will usually allow a patient to have balanced hearing, with speech perceived equally loud in the two ears. The eye and degree of hearing impairment may limit the benefits to be gained from a hearing aid. It is true that speech must be loud enough to permit the listener to understand to his full capability, but making speech increasingly louder will not necessarily lead to a corresponding improvement in discrimination because the hearing nerve has become less sensitive to the acoustic differences of speech sounds. Changes in circuitry of the hearing aid, specially designed ear moulds, and highly adjustable aids have greatly eased the initial learning process for many patients.

As you learn to discriminate between noise and speech and to identify various background sounds, you also will be able to ignore extraneous noises just as persons with normal hearing do. For maximum benefits lipreading rehabilitation should accompany the practice training in using the hearing aid.
Surgery is minor, and Baha recipients report a wide range of advantages over other hearing devices. The hearing device transmits sound vibrations through the titanium implant to the skull and the inner ear – where the hearing takes place. The BAHA sound processor is directly in contact with the bone of the skull through the titanium attachment allowing direct bone conduction (BC), the sound quality is significantly better than the bone conduction hearing aid, which was used in the past.
Traditionally, patients would need to use two hearing aids, one on the deaf side to pick up the signal and another hearing aid in the hearing ear (to take the sound from the other ear and place it in the hearing ear).
Recent studies have shown that patients prefer the sound and speech clarity achieved with the BAHA when compared to the CROS and the unaided condition. Since the BAHA sound processor produces such great quality sound, it is very rare for patients to want the procedure reversed.
If it is determined that tinnitus is being caused by a medical problem such as high blood pressure, ear infections, excessive ear wax, or even excessive use of aspirin or antibiotics, or drinking too much caffeine, the tinnitus can be medically treated. The Baha system, being considered a surgical procedure, is also covered by insurance and Medicare.
A hearing impaired person will often say,“I hear but I can’t always understand what I hear.” Because the prime function of an aid is to amplify sounds, some uses of these instruments continue to experience difficulty in understanding. While you are listening to one speak or to your radio or television in your own home, gradually turn up the volume control of your hearing aid until the sound is very loud.
It is suggested that you arrange to have a friend telephone you at a certain time each day for several days to help you become accustomed to the telephone procedure with the hearing aid.
BAHA is used to improve hearing in patients with chronic ear infections, congenital external auditory canal atresia (when the ear canal does not develop) or one-sided deafness who cannot benefit from regular hearing aids. In these cases or when the infection recurs with the use of regular hearing aids that are placed in the ear canal, the BAHA is a suitable choice for hearing improvement. There is no pressure on the skin with the use of the BAHA which prevents the pain and headaches caused by the bone conduction hearing aid. Tinnitus is typically observed by the person suffering from the condition rather than by others.
Still, if there are no specific medical issues involved and the tinnitus cannot be medically treated, simply knowing that no serious medical pathology such as a benign tumor or neurological degeneration exists can be a huge relief.
The use of hearing aids to amplify sound can help to create a sound-enriched environment and afford some relief.
The most important advantage, however, is that most hearing aid users are able to understand speech in a noisy background much better than when an aid is used only in one ear. Hearing aids that are placed in the ear canal may aggravate the infection by plugging the ear canal and contributing to increased humidity, which can allow bacteria to grow and cause infections. But if stress and anxiety resulting from tinnitus persists, there are options that can help provide relaxation and relief. Hearing aids provide access to sounds that have not been heard in some time giving the wearer more sound to pay attention to and also potentially covering up, or masking, the tinnitus that the person has been experiencing. Furthermore, using two aids results in a significant increase in the loudness of sound due to binaural summation. The hearing aid offers the user hearing that is short of normal acuity but more satisfactory than the uncompensated impairment. Some individuals, perhaps because of the severity of their loss or because of the nature of their hearing impairment, may require may weeks to learn to use the aid: even then they may never have a completely satisfactory adjustment. The advantage of the BAHA sound processor is that it transmits the sound directly to the hearing organ without going through the ear canal and middle ear.
Tinnitus can also be associated with ear infections, aging, excessive ear wax, high blood pressure, and sensory nerve disorders.
Others will find that the adjustment entails only minor problems and will be wearing the aid without any great difficultly within a few hours. Certain activities, such as smoking, drinking alcohol or caffeine, and taking excessive amounts of aspirin or antibiotics can also cause tinnitus. The BAHA sound processor does not cause the occlusion effect, feedback and whistling seen with conventional hearing aids that are placed in the ear canal.
