Sciatica is not a medical diagnosis, rather it is a term used to describe the patient’s symptoms that involve a radiating pain, tingling, and numbness along the sciatic nerve.  The sciatic nerve branches from the lower back through buttocks, legs and feet. The sciatic nerve can get irritated or compressed by various underlying causes, including herniated disc (or bulging disc), spondylolisthesis, spinal stenosis, piriformis syndrome, spinal tumor or spinal infection. When acupuncture is deemed applicable to treat this condition, an acupuncturist may determine the type of sciatica based on the patient’s body constitution.  In addition, acupuncture needles may be administered specifically to the affected nerve root and its branches. On the dorsum of the foot, between the first and second metatarsal bones, in the depression distal to the junction of the bases of the two bones, over the dorsalis pedis artery. BL60: Kunlun ?? On the posterolateral aspect of the ankle, in the depression between the prominence of the lateral malleolus and the calcaneal tendon. GB41: Zulinqi ??? On the dorsum of the foot, distal to the junction of the bases of the fourth and fifth metatarsal bones, in the depression lateral to the fifth extensor digitorum longus tendon.
Elden et al[1] 2005 published a randomised single blind controlled trial involving 386 pregnant women in the British Medical Journal (BMJ). Acupuncture was superior to stabilising exercisers in the management of pelvic girdle pain in pregnancy. The objective was to compare the efficacy of standard treatment for pelvic pain (a pelvic belt, patient education and home exercisers- for abdominal and gluteal muscles) to standard treatment plus acupuncture or standard treatment plus physiotherapy stabilising exercisers (for the deep lumbopelvic muscles). The study time frame consisted of one week which was used to establish a baseline, followed by six weeks of treatment.
Three physiotherapists gave standard treatment, two medical acupuncturists delivered the acupuncture treatment and two physiotherapists gave the stabilising exercisers. Pain was measured by a visual analogue scale and by an independent examiner before and after treatment. An alternative, safer method: Lift the muscle and insert the needle approximately 1 cun anteriorly or posteriorly into the muscle belly.
