Automatic thoughts are the thoughts that just pop into the client's head in any given scenario. Some therapists may also include a 4th column called 'Alternative Thought' and have the client develop a new thought that may result in a new and more positive behavior and feeling. From the initial session, the therapist will develop a case conceptualization, which is basically an understanding of how the disorder developed, and continue to expand on this throughout treatment.

Core beliefs are the most central beliefs that people have about themselves, others and the world around them. They are different from core beliefs in that they relate to the thinking that the client engages in on a regular basis and is likely not aware of at all. This column can help empower the client by providing a concrete tool to help restructure his own thoughts. If these thoughts are restructured through CBT, she would have quite a different emotional response to the same stressful situation. It is common in CBT to use what is known as a thought record to help clients understand the connection between thoughts and behaviors and feelings.
The therapist will draw 3 columns on a sheet and work with the client to fill in the blanks.
