Crash: First Air B732 near Resolute Bay on Aug 20th 2011, impacted terrain. A First Air Boeing 737-200, registration C-GNWN performing flight 7F-6560 from Yellowknife,NT to Resolute Bay,NU (Canada) with 11 passengers and 4 crew, went missing while on approach to Resolute Bay and was found crashed. 3 passengers survived with injuries and are reported in stable condition, 8 passengers and all 4 crew perished. Permanent link to this item in the archive.
EASA Airworthiness Directives Publishing Tool. Cracks on the forward face of the Rear Pressure Bulkhead (RPB) web have been discovered on three CL-600-2B19 aeroplanes in-service. This indicates that the existing inspection requirements of Airworthiness Limitation (AWL) task 53-61-153 mandated by AD CF-2005-13R1 are not adequate. But Air France A332 over Atlantic was all pilot error Investigation conducted by Airbus and Thales on the removed probes revealed oil residue between the stator and the rotor parts of the AoA vane position resolvers. This oil residue was the result of incorrect removal of machining oil during the manufacturing process of the AoA resolvers. At low temperatures, this oil residue becomes viscous (typically in cruise), causing delayed and/or reduced AoA vane movement. Crash: Asiana B744 near Jeju on Jul 28th 2011, fire in cargo hold. Boeing 747-400 freighter was carrying 58 tons of cargo including 0.4 tons of hazardeous materials like Lithium batteries, paint, amino acid solution and synthetic resin. The crew had reported the cargo on fire with Shanghai Center and was diverting to Jeju Airport when it crashed about 70nm west of the Island at 04:12L (19:12Z Jul 27th), 67 minutes after it had taken off Seoul. Flight 963 Dumping Fuel (DFW to Brazil) 7-24-11â Flight 963 from DFW to Sao Paulo, Brazil had to dump it's fuel about 14 minutes into flight. Seems they had a detrimental engine failure on their right side engine. They dumped fuel to try and lighten the aircraft to make it back to the landing strip. Incident: Pinnacle CRJ2 at Detroit on Mar 30th 2011, wheel well bin missing. The NTSB reported on Jul 22nd 2011 however, that the wheel well bin was found missing after the flight. The aircraft sustained no damage, no injuries on board or on the ground have been reported. The bin has so far not been located. This condition, if not detected and corrected, could reduce or block the oxygen supply, possibly resulting in injury to passengers when oxygen supply is needed. During production of passenger oxygen containers, the manufacturer B/E Aerospace detected some silicon particles inside the oxygen generator manifolds. Investigation revealed that those particles (chips) had chafed from the mask hoses during installation onto the generator outlets. An operator has reported a ground fire in the CL-600-2B19 aeroplane. The fire burnt an 18 inch hole through the left upper fuselage skin panel in the cockpit area. Crash: Silk Way IL76 near Bagram on Jul 6th 2011, impacted mountain. A Silk Way Airlines Ilyushin IL-76 on behalf of International Security Assistance Force (ISAF), registration 4K-AZ55 performing a freight flight from Baku (Azerbaijan) to Bagram Air Force Base (Afghanistan) with 9 crew and 18 tons of cargo (automobiles and trailers), was on approach to Bagram's Air Force Base, located about 22.5 nm north of Kabul, when radar contact was lost with the aircraft that was flying about 13.5nm from Kabul descending through 4000 meters/13000 feet at around 00:10L (19:40Z Jul 5th), no emergency call and no report about any anomaly had been received from the crew. The aircraft was located in flames on a mountain side near Akhshay in the Ghorband/Syahgerd district at 3800 meters of elevation. CF-2011-17 : Incorrect Assembly of Hydraulic Servo Actuators Transport Canada was advised that a quality escape by a supplier has occurred and a number of Bell Helicopter hydraulic servo actuators may have a loose nut, shaft, and clevis assembly. The loose connection is due to improper lock washer installation. This discrepancy is not traceable or identifiable except by inspection.
