A Wart can present as a solitary lesion or they can appear in clusters more commonly known as a mosaic. Over the years I have seen many doctors who do not specialize in foot problems, misdiagnose warts.
The picture above is a solitary wart located on the bottom of the foot just behind the third toe. There are a million wart medications on the market, most of which are not strong enough for plantar warts (because the wart is beneath the skin). For those who have many warts numbering in the dozens if not hundreds, simple acids or surgery will not work due to the large amount of warts that need to be treated. In these cases cimetidine (Tagamet, used for stomach ulcers) has been shown to be effective in eliminating cutaneous lesions such as warts. Vitamin A in large oral doses, by prescription only, has also been shown to be effective in eliminating large areas of wart. Recently a natural phytochemical known as Diindolylmethane has become popular in treating warts. Vitamin D which is currently being touted as an effective treatment for many medical conditions, has been shown to be effective for warts. It should be mentioned that any wart that seems to keep recurring should be biopsied to make sure it is not something other than a wart. I had been experiencing extreme heel and sole pain for about six months and had to take extended breaks off my feet many times a day as well as regular doses of Ibuprofen.
I have been experiencing foot pain of various sorts and am working to figure out what it is.
Viral warts are benign proliferations of the skin and mucosa caused by infection with Human Papilloma Virus (HPV); more than 150 subtypes of the DNA virus are known.
Intraoral warts are usually misplaced genital types but those on the lips may also be common or filiform variants.
Immunosuppression can result in malignant transformation of warts on non-genital sites, most commonly ten or more years after organ transplantation. Rarely, longstanding non-genital warts can cause verrucous carcinoma (large solitary well differentiated squamous cell carcinoma usually found on plantar surface).
In children, even without treatment, 50% of warts disappear within 6 months; 90% are gone in 2 years. Daily application of paints or gels containing keratolytics such as 15 to 60% salicylic acid. Cryotherapy with liquid nitrogen (cotton bud or cryospray) to wart and 1-2 mm rim of normal tissue. Podophyllin (podophyllum resin), a cytotoxic agent present in a resin extracted from the May Apple plant and available as over-the-counter ointment or paint. Podophyllotoxin (refined podophyllin), applied to genital warts for 4-day courses each week for several weeks. Trichloacetic acid 80% applied weekly by the physician, resulting in immediate superficial (and sometimes deep) tissue necrosis.
Contact allergens such as dinitrochlorobenzene (a mutagen) or dibutyl squaric acid, which cause local inflammation.
Laser vaporisation using carbon dioxide laser, used for diffuse warts or deep plantar warts. Most of the warts that occur in childhood will eventually disappear if untreated; however this is not true for all warts. Imiquimod (Aldara) is an immune system booster available in a 5% cream that is FDA-approved for the treatment of genital warts, actinic keratoses (pre-cancers of the skin) and superficial basal cell carcinomas (common skin cancers).
When combined with liquid nitrogen, salicylic acid or occlusion imiquimod may be useful for common and plantar warts.


Candida and mumps antigens are FDA-approved drugs used in skin testing that have been used off-label to make warts disappear.
Salicylic acid is a beta hydroxy acid available in a paste, pad (Mediplast) or solution form. Salicylic acid is useful in children but not recommended for pregnant women because of a slight risk of birth defects. Trichloroacetic acid is a caustic acid that is used to destroy superficial skin tumors and warts. After the wart is anesthetized with an injection of lidocaine, an electric needle is used to burn it, followed by scraping with a sharp instrument called a curette. Bleomycin is an anti-cancer medicine that is used in the off-label treatment of difficult and resistant warts. Some patients have experienced persistent finger pain, scarring and intolerance to cold after bleomycin therapy.

It must cause minimal pain to the patient whilst the plantar wart treatment process is in progress. He is a 35 year old male he presented with one plantar wart on the Left foot behind the great toe the wart has had plantar wart treatment before as follows. He went to a Podiatrist and had the wart burnt with a solution of Salicylic Acid for several months there was no noticeable result and in fact the plantar wart became more painful. He then decided to seek a second opinion and went to a second Podiatrist he was treated further with more acid treatment and again there was no result. He then decided to visit a General Practitioner who initially froze the plantar wart with liquid nitrogen after several months when there was still no result he decided to apply more acid to the area suggesting it was “almost gone” but a month later it was still there and causing pain. This patient was very sceptical after spending a considerable amount of both time and money to find a plantar wart treatment that worked.
We initially asked the patient to return in 1 week as the wart was badly damaged with the continual use of acid this time allowed the Wart to dry out.

A Wart over a bony prominence such as the ball of the foot should never be surgically removed because if a scar develops it may be as painful as the original wart.
If you have warts on other parts of your body, they too have to be treated at the same time as the plantar warts otherwise you will not get rid of the warts on your feet. Although not common, a wart may be mistaken for a squamous cell carcinoma or a wart can actually deteriorate into what is known as verrucous carcinoma.
Viral warts are particularly common in childhood (affecting about 15% of school children) but may arise at any age. Persevere for twelve weeks or longer; up to 70% of warts will resolve with wart paints, and removing the keratinised surface relieves discomfort.


The hard skin should be rubbed away from the wart surface with a piece of pumice stone or emery board. In some instances, the wart may recur or fail to respond after the initial treatment with liquid nitrogen. After topical anesthesia with cream or a cold spray, the medicine is injected directly into the wart. Topical salicylic acid under occlusive dressing can potentially increase the risk of systemic toxicity and teratogenic (birth defect) risk.
If used on soft mucosal skin it is applied directly to the wart in the office and washed off at home 3 hours later. Bleomycin can be painful and an analgesic may be required both during and for several days after treatment. It is predominantly used in genital warts called condyloma but may be used in common warts in combination with other treatments. Patients find it extremely difficult to carry on their normal day to day activities without almost constant pain.
Walking barefoot on the pavement is enough to develop the micro trauma that will enable the Human Papilloma Virus to inoculate the foot with a plantar wart.
Some of these methods include duct tape, acids, freezing with liquid nitrogen, banana skin and the list just goes on and on. Plantar wart treatment should commence as early as possible otherwise the plantar warts may spread and grow in size making any form of treatment more difficult.
I have to say though, after using them for just four days, I have experienced grand relief from my foot pain. The wound heals in about two weeks; even then 20% of warts can be expected to recur within a few months.
There are about 120 kinds of virus present out of which few are responsible for these warts. The wart should be soaked for 10 minutes in warm water and the surface of the wart should be abraded with a pumice stone or a file.
Some patients will get this micro trauma from the non-slip surface around swimming pools and this is also a good place to be inoculated with the plantar wart virus.
After 9 short weeks the Plantar Wart resolved there was no further pain from this lesion, the patient was extremely happy.
Even the very first day, I was able to do a lot of work while on my feet with at least a 75% reduction of pain. Side effects include pain, injury to underlying tendons and superficial nerves (avoid the sides of digits), postinflammatory depigmentation, scarring and recurrent warts (often as a ring around the treated site).
It works better if covered with adhesive tape (particularly recommended when the wart is on the foot). Occasionally, cantharidin will produce a doughnut wart, a central clear zone surrounded by a ring of new warts.
As other people with warts already on their feet will slough small bits of tissue infected with the wart virus the combination of micro trauma and infected tissue leads to the development of Plantar Warts.
The warts are located on the bony parts like ball of the foot or in the heels which receives the total pressure of the body.
