There is a broad band of connective tissue that originates across the entire bottom of the heel called the plantar fascia that runs from the bottom of the heel to the base of the toes and helps support the arch. Heel pain is most often caused by plantar fasciitis -- a condition that is sometimes also called heel spur syndrome when a spur is present.
People with plantar fasciitis often describe the pain as worse when they get up in the morning or after they've been sitting for long periods of time. In addition, diagnostic imaging studies such as x-rays, a bone scan, or magnetic resonance imaging (MRI) may be used to distinguish the different types of heel pain.
The Plantar Fascia is a non-elastic band of fibrous tissue that is connected to your heel bone and runs through the bottom of your foot spreading into five bands and attaching to the metatarsal joints of the forefoot. At home we recommend that you rest the feet as much as possible, and replace high-impact exercise with lower impact activities while you are still having heel pain. Hot and cold therapy is also very beneficial to reduce some pain, especially after you get home from work. Stand with your feet together, then rotate your knees outward while you are attempting to stay on the floor using the muscles of your feet. Other conditions that can produce heel pain include a fracture of the heel bone, slivers or embedded objects, fissures or severe cracks in the skin, plantar warts and problems with the fatty padding.
It is important to attend the foot clinic to make sure you know what is making your heels sore. The minute they put their foot to the floor after sleeping all night, riding in a car for hours, sitting at a desk all day, or any long period of rest, their heel hurts. This pulls on the plantar fascia and since it’s already inflamed or damaged it makes the plantar fasciitis worse and causes more pain.
I recommended many of the remedies that came down the pike over the years, but after treating over 12,000 patients with the condition I concluded that the best approach is to start conservatively before resorting to more extreme methods such as cortisone shots, shock therapy or even surgery. Heel pain may also be due to other causes, such as a stress fracture, tendonitis, arthritis, nerve irritation, or, rarely, a cyst.
Plantar fasciitis is an inflammation of the band of tissue (the plantar fascia) that extends from the heel to the toes.


For example, people who have problems with their arches—either overly flat feet or high-arched feet—are more prone to developing plantar fasciitis. Throughout this process the surgeon rules out all the possible causes for your heel pain other than plantar fasciitis.
Sometimes heel spurs are found in patients with plantar fasciitis, but these are rarely a source of pain.
Putting an ice pack on your heel for 10 minutes several times a day helps reduce inflammation.
Wearing supportive shoes that have good arch support and a slightly raised heel reduces stress on the plantar fascia. Nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, may help reduce pain and inflammation.
In some cases, corticosteroid injections are used to help reduce the inflammation and relieve pain.
If, after several months of non-surgical treatment, you continue to have heel pain, surgery will be considered.
The most common time for experiencing heel pain is first thing in the morning, after you get out of bed or after resting, and after you have been sitting for a while or driving. The heel pain comes from pulling of the fascia from the heel bone, causing pulls and tears, inflammation and pain.
At the Kingston Foot and Ankle Clinic, we will take plaster of paris casts of both feet in the non-weight bearing position. The laser is used to break-up or disperse inflammation at the site of the tear to reduce pain and accelerate the healing process.
You should put the feet in moderately warm water for 2-5 minutes followed immediately with 2-5 in (5-12 cm) of cold water (or use ice packs).
Because there are several potential causes, it is important to have heel pain properly diagnosed. In this condition, the fascia first becomes irritated and then inflamed-resulting in heel pain.


For some people the pain subsides but returns after spending long periods of time on their feet.
You can also get some micro-tears along the fascia, which could result in arch pain and forefoot pain. This is a painless procedure where you sit comfortably in the chair and the pedorthist applies the light therapy and then the laser to the affected area.
Repeating this exercise will take some pain away in acute cases that are very painful, and is also beneficial in prevention. The pain diminishes after a few minutes of walking around, but then gets worse again the longer they stay on their feet.
A foot and ankle surgeon is best trained to distinguish between all the possibilities and determine the underlying source of your heel pain.
