This Statistical Brief presents data from the Healthcare Cost and Utilization Project (HCUP) on ADEs in 2004. Hospitalized patients with ADEs were on average about 62 years of age, compared with 47 years for all inpatients. While the top five causes of adverse effects were the same as the most common ADEs overall, the most common causes of drug poisoning were benzodiazepine tranquilizers, opiates, aromatic analgesics (such as acetaminophen), antidepressants, and anticonvulsants.
Consistent with the findings on age, figure 3 shows that most patients with adverse effects were covered by Medicare (58.5 percent). Figure 4 summarizes information on the top 10 specific causes of all ADEs, which comprised over half (53.5 percent) of all ADEs. Table 2 provides information on general causes of ADEs, as well as details on specific causes for ADEs that account for at least 10,000 cases in U.S. Hormones and synthetic substitutes ranked second, accounting for a total of 183,200 hospital stays. The fifth most common cause of ADEs were systemic agents (which include antiallergic and antiemetic drugs, enzymes, and vitamins, among others), accounting for a total of 127,400 ADE-related hospital stays. Figure 5 illustrates the top five specific causes of ADEs for the adverse effects and poisoning subgroups. The estimates in this Statistical Brief are based upon data from the HCUP 2004 Nationwide Inpatient Sample (NIS).
Total hospital charges were converted to costs using HCUP Cost-to-Charge Ratios based on hospital accounting reports from the Centers for Medicare and Medicaid Services (CMS).2 Costs will tend to reflect the actual costs of production, while charges represent what the hospital billed for the case.
The principal diagnosis is that condition established after study to be chiefly responsible for the patient’s admission to the hospital. ICD-9-CM is the International Classification of Diseases, Ninth Revision, Clinical Modification, which assigns numeric codes to diagnoses. Because the NIS is based on a sampling frame of statewide data-collection organizations, we examined the coding of ADEs by state and found considerable variability across states in the percentage of all records that have an ADE code. The HCUP Nationwide Inpatient Sample (NIS) is a nationwide database of hospital inpatient stays. HCUP is a family of powerful health care databases, software tools, and products for advancing research. AHRQ welcomes questions and comments from readers of this publication who are interested in obtaining more information about access, cost, use, financing, and quality of health care in the United States.

Bacterial Infection Lawyer Serving Rhode Island, Massachusetts & New YorkAntibiotics can be an extremely effective treatment against various infections and viruses. Yes, we have tons to do in so little time, but that doesn’t excuse us from committing errors in the clinical area especially when administering medications.
It may seem cliche to some already, but reviewing the 5 Rights is a must when administering medications to patients, may they be oral, intravenously, topical and many others. If you seem to be doubting something, let’s say a questionable order of dosage of a certain drug, never hesitate to clarify.
As a nurse on the incoming shift, always review all new orders to ensure that each patient’s order is noted and transcribed correctly on the physician’s order and the medication administration record (MAR) or the treatment administration record. Always read back and verify medication orders given verbally or over the phone to ensure the ordered medication is transcribed correctly.
Those that need to be refrigerated must be kept in the fridge to maintain efficacy, while those medications that should be kept at room temperature should be stored accordingly. Currently an Intensive Care Unit nurse, pursuing a degree in Master of Arts in Nursing Major in Nursing Service Administration. Too many people on Rhode Island's streets and highways fail to devote their full attention to the task of driving. If any one of these or other driving behaviors results in an accident, the accident victim can claim compensation for medical costs, lost income, pain and suffering, and other economic and noneconomic losses.
This publication provides national estimates on the types of patients seen with ADEs in U.S. About 59 percent of all patients with ADEs were female—a similar distribution to all hospital stays. Hospital costs were similarly lower for those with poisoning ($6,000 per stay versus $10,500 for cases with adverse effects).
Corticosteroids were the most common specific cause of ADEs, accounting for 141,000 hospital stays, or 10.3 percent of all ADEs.

This category includes such drugs as insulin for diabetes; and corticosteroids (such as prednisone) used to treat arthritis, asthma, allergic reactions, hepatitis, ulcerative colitis, and other inflammatory conditions.
This category includes high blood pressure medications (antihypertensives), cardiotonic drugs such as digitalis, and other drugs specifically for the heart.
Antineoplastic and immunosuppressive drugs were the most common specific cause in this category. HCUP data include OB-GYN, ENT, orthopedic, cancer, pediatric, public, and academic medical hospitals. For each hospital, a hospital-wide cost-to-charge ratio is used because detailed charges are not available across all HCUP states. Secondary diagnoses are concomitant conditions that coexist at the time of admission or that develop during the stay. Sponsored by the Agency for Healthcare Research and Quality (AHRQ), HCUP includes the largest all-payer encounter-level collection of longitudinal health care data (inpatient, ambulatory surgery, and emergency department) in the United States, beginning in 1988.
We also invite you to tell us how you are using this Statistical Brief and other HCUP data and tools, and to share suggestions on how HCUP products might be enhanced to further meet your needs. For 13,300 cases with neuropathy or dermatitis ADE codes, the ICD-9-CM codes do not identify whether the origin of the ADE was poisoning or adverse effects due to drugs properly administered.
This is based on a total of 1,364,100 ADEs in 1,211,100 hospital stays with at least one ADE recorded. There are certain circumstances, however, where antibiotics can be overused and can eventually do more harm than good. However, adverse drug events (ADEs), either due to inherent side effects or from inappropriate use, are serious patient safety concerns.
The vast majority of ADEs (90.3 percent, or 1,093,600 stays) represented adverse effects of drugs. Among cases with poisoning, 80 percent were admitted through the emergency department, compared with 62.9 percent of cases with adverse effects.
Patients with adverse effects tended to be older (63 years of age average) while patients with poisoning were more likely to be younger (47 years). Anticoagulants (blood thinners) ranked second, accounting for 117,700 hospitalizations, or 8.6 percent of all ADEs.
They exclude long-term care, rehabilitation, psychiatric, and alcoholism and chemical dependency hospitals, but these types of discharges are included if they are from community hospitals. Hospital charges reflect the amount the hospital charged for the entire hospital stay and does not include professional (physician) fees. All-listed diagnoses include the principal diagnosis plus these additional secondary conditions. The NIS is a sample of hospitals and includes all patients from each hospital, regardless of payer.
For 13,300 cases with neuropathy and dermatitis ADEs, a specific cause cannot be determined. A great deal of care must be taken to ensure that patients are given the right amount of a drug and that treatment is stopped before it begins to compromise the patient's health. When antibiotics are overused, it can lead to an infection by a bacteria known as clostridium difficile, also referred to as C-difficile or C-diff.

ADEs are split into two broad categories—adverse effects of drugs properly administered and drug poisoning (accidental overdose, wrong drugs given or taken in error, or drugs taken inadvertently). These are drugs properly administered in therapeutic and prophylactic dosages but which cause adverse reactions, including allergic or hypersensitivity reactions. Compared with all inpatients, those with adverse effects were predominantly in the 65+ age group, while patients with poisoning tended to be younger, primarily in the 18–64 age groups. Antineoplastic (anticancer) and immunosuppressant agents (used to prevent rejection of organ transplants) accounted for 116,400 hospital stays, or 8.5 percent of all ADEs.
Analgesics, antipyretics, and antirheumatics were the most common general cause of ADEs, accounting for a total of 192,200 stays across the adverse effects and poisoning subgroups. It is drawn from a sampling frame that contains hospitals comprising 88 percent of all discharges in the United States. These infections can lead to a broad range of health complications and, in extreme cases, can be fatal.

Everything must be done accurately and things must be considered meticulously. One single mistake and you put your patient’s safety in danger. When documenting, include proper medication labeling, legible documentation, or proper recording of administered medication.
It is not possible to distinguish ADEs that occur in the hospital from those that originate outside the hospital, nor is it certain that all ADEs are captured in administrative data.
ADEs are further broken down into general drug categories as well as more specific categories. Most remaining ADEs were drug poisoning, which involve accidental drug overdose, wrong drugs taken or given in error, or drugs taken inadvertently. Opiates (such as codeine and morphine for pain control) were the fourth most common cause of ADEs, accounting for 79,900 stays, or 5.9 percent of all ADEs. The vast size of the NIS allows the study of topics at both the national and regional levels for specific subgroups of patients. Doctors should be aware of the risks associated with excessive antibiotic use and may be held accountable if patients acquire a C-diff infection. At the law firm of Vincent D.

Nonetheless, these data can provide some insight into the frequency and types of ADEs seen in U.S.
Care was taken to exclude hospital stays associated with illicit drug use or with evidence of intentional harm, self-inflicted or otherwise. Other specified analgesics and antipyretics (such as pentazocine) ranked fifth with 60,200 stays (4.4 percent of all ADEs).
