Everyone has loads of questions about their bodies and climbing and all the associated bits that go along with it. Robert Bradshaw-Hilditch and Gary Gibson (yes, THAT Gary Gibson) are both podiatrists who have been conducting some brilliant studies in collaboration with Staffordshire University regarding the forces that are exerted through climbing shoes, and where. Their research has found that the forces when front pointing on the hallux (big toe) during climbing causes more force through the metatarsal head, and puts the plantar fascia under tension. During edging, the forces through the hallux again puts most of the force through the hallux metatarsal head, and stresses the plantar fascia, but also places the foot in a supinated position. This is just the tip of the iceberg for this research, and Rob and Gary are looking at expanding this much further, as this was only looking at the plantar aspect of the foot in an indoor climbing situation. This means that the feet are more prone to ankle injuries due to the supinated position (see below), and this increase in pressure on the plantar fascia could cause plantar fasciitis (see below as well). Hallux valgus is defined as a 20 degree difference between the axis of the first metatarsal and the axis of the proximal phalanx of the toe, and was noted in both feet in 53% and in one foot in 20% of climbers participating in the sport for more than 5 years and climbing UIAA degree IX.
Bunions are not actually caused by wearing tight shoes, but climbing shoes can worsen the deformity. Treatments include wearing bunion pads, orthotics or different footwear and pain killers, or there is the surgical route if the pain is severe. Rock climbing has been found to have a beneficial impact on longitudinal arch of the foot (due to strengthening), but does cause an increase in frequency in transverse arch disorders such as tansversal platypodia (flat foot),and an increased frequency of abnormal toe-to-surface adhesion.
Both these problems affect the frontal areas of foot, caused by climbing footwear – changes in the biomechanics of the foot, can cause weakness in muscles controlling 1st metatarsal head extension.
Ankle sprains are an injury that don’t just affect rock climbers, as you may well realise.
Therefore, the preventative measures you can do to improve the acting of your muscles around the ankle would be to improve the proprioception of the ankle (knowledge of where your body is in space). Early stage treatment for an ankle sprain would be to follow the management of acute injuries, along with maintaining range of movement in a non-weight bearing manner. Plantar fasciitis is heel pain that is caused from an inflammatory process of the connective tissue, the plantar fascia. The treatment is normally rest, ice, reduce inflammation and swelling, calf stretches, and finally, correcting what caused the problem in the first place, be that poor footwear, muscular imbalances etc.
So, an ankle fracture, as you’d have guessed, is when a bone involved in the foot or ankle gets broken.
The time frame and approach is entirely dependant on where the fracture etc is, and what other structures are involved. There are also other non-musculoskeletal problems hat can occur with the foot, such as corns, cuts, toe infections etc which just needs you to look after your feet!

As previously mentioned, use of the wobble board or cushion can help prevent some ankle injuries. Ensuring you have appropriate sized climbing shoes, or if not possible, remove them at all opportunities, or alternate your shoes for different routes.
To avoid some ankle injuries, have some (decent!) spotters when bouldering, and try to have dynamic belay techniques when roped climbing, to avoid clattering into the rock and giving you time to slow down the motion with your upper legs rather than at the ankle! Much like the foot fetish itself, aesthetic foot surgery has become somewhat controversial, and, in some arenas, taboo or pilnapadystes pozymiai. An ankle sprain is caused by an unnatural twisting or force on the ankle bones of the foot, which may result in excessive stretching or tearing of one or more ligaments on the outside of the ankle.
Primary symptoms of ankle sprains are pain following a twist or injury, swelling, and bruising. To prevent an ankle sprain, try to maintain strength, balance, and flexibility in the foot and ankle through exercising, stretching, and wearing well-fitted shoes. A fracture is a partial or complete break in the bone caused by a structure that is stronger than the bone can withstand. Your child will need to see a doctor and have a repeat X- ray within a few weeks of the injury so that the fracture can be checked.  Normal activities may be resumed after discussion with your doctor. Everyone is different and some injuries may heal quicker than others, depending on the fracture’s location and severity. Anasarca is generally due to water retention, blood vessel defect, and lymphatic obstruction. In the presence of hypoalbuminemia as seen in nephrotic syndrome or other disease entities, anasarca occurs as a result of low osmotic pressure in the capillaries. As a result, Kimmelstiel-Wilson nodules develop, obstructing the glomerular blood flow and damaging the nephrons (kidney cells). As a result, there will be decreased blood volume in the arteries because of sluggish blood flow.
But if we are talking about generalized edema or anasarca, then we have a big problem as it mirrors a serious underlying condition. If the albumin is normal, a common underlying cause is heart failure and this can be observed in patients with jugular venous distention (JVD) on the side of the neck or decreased cardiac output (CO) as the heart fails to pump sufficient blood to cater the needs of the entire body. Anasarca is not a disease entity by itself but rather, it is a sign that something severe is going on inside the body.
Bed rest is required for faster recovery especially if the cause of anasarca is congestive heart failure or liver cirrhosis. For swelling of the lower extremities, lie down flat on your back and elevate your legs and feet against the wall.
Diuretics are used for congestive heart failure, pulmonary edema, and excessive sodium ingestion.
For liver and kidney failure, hemodialysis may be used to get rid of the excessive interstitial fluid. In congestive heart failure, do not overuse diuretics as it may lead to further decreased cardiac output, azotemia (accumulation of proteins in the blood), and hypokalemia (decreased serum potassium levels).
In anasarca caused by liver diseases, spironolactone (a potassium-sparing diuretic) is the diuretic of choice. There will be continuous movement of lymphatic fluid in the bloodstream and due to any reason, if it gets blocked, it may give rise to lymphedema. Here the veins flow the blood moving in one way direction thus keeping the blood flow insufficient.
They would get nerve problem called diabetic neuropathy causing foot infection leading to formation of sores and blisters causing nerve damage.
There is every chance for fluid accumulation in the feet when kidneys are not functioning properly or when there is leakage in liver or when there is sudden heart failure. Using antidepressant drugs like norpramin, aventyl, amitriptyline, endep and nardil can cause swelling of feet. So to help answer some of these questions we’ve teamed up with top physio, Thomas Bond, who over the next few months is going to attempt to explain some things about our bodies and climbing.
Outside of work, I have a special interest in rock climbing injuries and enjoy teaching others about injuries and rehab for them. The forces being placed through the metatarsal head will change the biomechanics of the foot through the strength of certain muscles, and could cause problems with the arch of the foot (see below) and these problems will surpass what happens on the rock alone. There is no physiotherapy intervention that can help here, therefore prevention is the best cure, by avoiding wearing tight shoes and decreasing the effect of escalating the problem.
This is because of change in the sensation in the feet (neuropathy) that can be caused by diabetes, and can cause much more serious foot problems!

One of the most common concerns is the likelihood of improvement compared to the risk involved. The severity of the sprain can impact the degree of damage as well as the type and duration of treatment. There are different types of fractures and the treatment depends on the type of fracture sustained.
A plaster cast will be applied to keep the limb in the right position and protect it whilst it heals. The presence of marked swelling of the entire body signifies a severe underlying condition.
Diabetes mellitus is very rampant and as the morbidity rate increases, cases of diabetic nephropathy as one of its complications also increase. But because these are spilled into the urine, there will be less proteins in the plasma, decreasing the osmotic pressure. The disease worsens, stimulating neurohumoral excitation, causing sodium retention in the kidneys and expanded plasma volume. If you have edema, the first question to be asked is “Is it localized or generalized?” If it is localized, it may be due to venous obstruction, lymphatic obstruction, or local injury. Thiazides or loop diuretics may be added but giving diuretics should be closely monitored because overuse may backfire to a worse complication like hepatic encephalopathy. In case of abdominal pain, nausea or vomiting symptoms, then you should consult your doctor immediately.
Sometimes, the ligaments may not sit properly when stretched beyond its limit for some reasons.
Due to this process, the valves get damaged and in turn blood drains down the veins causing accumulation of fluid in lower legs, causing swollen feet. They would not feel any sensation in their feet and hence swollen feet during diabetes is truly a matter of concern.
You need to consult your doctor when you have symptoms of fatigue, loss of breath, sudden weight gain and loss of appetite along with swollen feet. I’ve attended both BMC Climbing Injury Symposiums in 2010 and 2012, and in 2012 presented my dissertation on grip strength in rock climbers. Therefore, the approach for a fracture is surgical intervention, or conservative treatment (which normally involves just casting the foot and ankle in plaster and waiting it to heal).
Serious ankle sprains, particularly among competitive athletes, may require surgery to repair and tighten the damaged ligaments. On some occasions, treatment of a fracture may require surgery and a short stay in hospital. In contrast with the word “swelling,” the whole body is grossly edematous and not just a certain part.
If osmotic pressure is low, fluid leaks out or extravasates into the interstitium, hence the edema. In this condition, there is increased capillary permeability and obstructed lymphatic vessels, resulting to extravasation of fluid into the subcutaneous tissue.
The most convincing among them is the peripheral arterial vasodilation hypothesis suggesting that ascites is caused by portal hypertension which leads to vasodilation. So more often than not, there is nothing to worry about as it will subside as the obstruction is removed and as the injury heals. If anasarca is due to hypoalbuminemia, the patient may either have severe malnutrition, liver cirrhosis, or nephrotic syndrome. Sometimes, this condition can become fatal if there is no absolute blood flow from the heart to the feet area.
In anasarca, the decreased capillary osmotic pressure is systemic, giving you a generalized edema or swelling. These two affect the glomeruli of the kidneys by increasing the thickness of its basement membrane, causing the glomeruli to enlarge.
As the proteins continue to spill into the urine, more nephrons become destroyed, worsening the proteinuria, and the cycle goes on and on until the condition progresses to renal failure. You can also apply ice packs or keep the leg in elevated position for reducing inflammation.
