Using psychological theory to understand the clinical management of type 2 diabetes in Primary Care: a comparison across two European countries. Background: Long term management of patients with Type 2 diabetes is well established within Primary Care. City Research Online is powered by EPrints 3 which is developed by the School of Electronics and Computer Science at the University of Southampton. Is the increased total mortality and sudden death risk associated with the intensive glycemic treatment strategy (ACCORD – VADT) linked to hypoglycemia ? Although metformin is first-line therapy for patients with type 2 diabetes, glycemic control fails with metformin alone in many patients.
However, despite extensive efforts to implement high quality care both service provision and patient health outcomes remain sub-optimal. Bernard Charbonell (University of Nantes, France) discussed new options in diabetes management. An open-label randomized controlled trial was conducted, which compared add-on exenatide with add-on glimepiride in patients who had failed to achieve glycemic control with metformin alone. We recognised that there was underlying moderate-severe insulin resistance affecting muscle and the liver, and this, coupled with beta cell failure, formed the classical triumvirate (1).

March 12, 2016 by Marie Veselsky MS, RD, LD, BC-ADM As a Registered Licensed Dietitian, I am always going to recommend a more holistic approach such as diet and lifestyle changes first, or in addition to any prescription drug you decide to take.
Several recent studies suggest that psychological theories about individuals' behaviour can provide a valuable framework for understanding generalisable factors underlying health professionals' clinical behaviour.
The insulin resistance in muscle primarily was responsible for the excessive postprandially rise in plasma glucose concentration, while insulin resistance in the liver, in combination with accelerated gluconeogenesis, resulted in an excessive rate of hepatic glucose production which led to an increase in the fasting plasma glucose concentration (2-4).

However, we did not observe a relationship between health professionals' intentions and our proxy measure of team behaviour. February 7, 2015By Marie Veselsky, RD, LD, CDE, BC-ADM Diet and exercise are most effective for preventing pre-diabetes from becoming type 2 diabetes.

Significant methodological issues were highlighted concerning the use of models of individual behaviour to explain behaviours performed by teams. A major study in 2002 found that the combination of diet and exercise reduced the odds of pre-diabetes becoming diabetes by 58%, compared with 31% among those using the common diabetes drug metformin. In order to investigate clinical behaviours performed by teams it may be necessary to develop measures that reflect the collective cognitions of the members of the team to facilitate the application of these theoretical models to team behaviours.
