Hadas Rabinovitz, from the Hebrew University of Jerusalem in Rehovot, Israel, and colleagues assessed blood glucose control and hormone profile in 59 adults with type 2 diabetes who were randomly assigned to either a big or small balanced breakfast group. The big breakfast contained one third of the daily calories that the diabetic patients would have, with a higher percentage of protein and fat, while the small breakfast contained only 12.5 percent of their total daily energy intake.
After 13 weeks, significantly larger reductions in blood glucose levels and systolic blood pressure were reported among those in the big breakfast group, as well as greater improvements in fasting glucose and reductions in hunger levels during the day. The researchers also found that one third of the big breakfast eaters were able to reduce their dependency on diabetes drugs, whereas 17% of the small breakfast group had to increase the amount of medication needed during the trial. For example, take some deep breaths every time you sit down to eat a meal or to catch up on email.
I have met patients who have underlying worries about the long-term effects of their high blood sugar, but it just never seems like the right time to change their way of coping. Start with the most intense sources of anxiety at the top of the list and the least feared situations, events or people at the bottom.  Work your way up the list gradually, gathering evidence about your ability to confront yours fears until you are able to tackle the most intense ones on the list. For the sub-study, Oramed plans to enroll 30 Type 2 diabetic patients in a controlled in-patient setting for up to one week, with data likely available in four-to-five months. I work on gathering evidence to develop new, more balanced beliefs with patients who are overcome with anxiety about hypoglycemia. I have seen patients reduce their levels of anxiety and greatly improve their quality of life.  It is absolutely worth the effort. I don’t care about using another test strip it if means my blood sugars are better controlled! It shows the higher sensitivity of IGT over IFG for predicting progression to type2 diabetes. Screening by the criteria for IFG alone would identify fewer people who subsequently, progress to type2 diabetes than would be the oral glucose tolerance test.
