So let’s dive right back into what is metabolic syndrome and how it links diabetes and hypertension.
It’s always a good idea to schedule an appointment for your annual physical first thing in the morning. The goal of the treatment for metabolic syndrome is to reduce the complications that it can lead to- such as diabetes and cardiovascular disease. When diet and exercise fail to bring the blood pressure down to normal range, your healthcare provider will opt to start you on medications. As always, I welcome your comments, feedback and any suggestions you have for upcoming topics.

Next post: BORDERLINE DIABETES – IS THERE A DIFFERENCE BETWEEN PREDIABETES AND TYPE 2 DIABETES? Knowing and understanding the results from a fitness assessment may potentially save your life!
After receiving your fitness assessment, you find out that you can a high resting heart rate, high blood pressure, low cardiovascular fitness, low muscular endurance, or are a little overweight. Also with physical activity you can improve your glucose management, which will decrease your risk for developing diabetes. A pair of studies published last week in Diabetologia, the journal of the European Association for the Study of Diabetes, concluded that taking blood pressure medications at night reduced the risk of type 2 diabetes by 57%. And while hypertension itself is considered a risk factor for type 2 diabetes, research has shown that some blood-pressure medications including diuretics and beta-blockers, especially when taken without other types of blood pressure drugs, may actually promote type 2 diabetes, especially in people who face an increased risk of the disorder. Meanwhile, weight loss and regular physical activity remain the most effective ways to prevent and improve both hypertension and diabetes. It is predicted that by 2025, 1.56 billion people worldwide will have hypertension and that by 2030 an estimated 366 million people will have diabetes. Ulcers occur when the skin breaks down allowing air and bacteria to get into the underlying tissue. Associated symptoms of a venous leg ulcer are caused by blood not flowing properly through your veins. Years of research have shown that the usual causes of leg ulcers are not a problem with the skin itself, but rather with the underlying blood supply to the skin.
Because blood supply to the skin is crucial, ulcers can occur as a result of poor circulation and so it is also mostly associated with disorders that affect circulation, such as diabetes, rheumatoid arthritis and hypertension. Diagnosis is usually made based on symptoms, location and the way the surrounding skin of the ulcer looks. Leg ulcers usually occur in the elderly more so than any other age group, due to poor circulation in aging limbs. Two conditions that add to the complications of leg ulcers in the elderly are obesity and diabetes. Venous (Varicose) Ulcers mostly occur due to improper functioning of the valves connecting the superficial and deep veins.
Arterial (Ischemic) Ulcers are caused by poor blood circulation as a result of narrowed arteries or by damage to the small blood vessels from diabetes. Treatment for leg ulcers should include weight loss if you are overweight and regular exercise to promote good circulation. Once the causes of leg ulcers are under control, (for example the blood sugar level in diabetes) the ulcer should heal by itself.

If an underlying disease is one of the causes of leg ulcers, it's important that it is treated - for example hardening of the arteries.
Sit with your legs raised whenever you have the opportunity - above heart level if possible. If your work requires a lot of standing or sitting, try to vary your stance as much as possible. Acute Hospital Discharge Services — patient monitoring programs used to reduce the number of early re-admissions to hospital. Medication Adherence Programs — to support patients’ compliance with their medication plans.
S3 Group has designed some of the world’s leading Remote Patient Monitoring services and solutions across multiple geographies. Since 2011, S3 Group and consortium partners have managed a telemonitoring service in Northern Ireland supporting over 3,500 people a year and helping to reduce the pressure on front-line services by enabling people to be cared for in the community. By using advanced technology in the right way, the service enables patients and carers to monitor their condition at home, leading to earlier intervention and reducing admissions to hospital. TelemonitoringNI is the first end-to-end clinically led remote monitoring service to provide an integrated model of care. Various patient services are provided, such as early discharge monitoring and self-management through the provision of flexible monitoring periods, configurable monitoring plans and capturing of monitoring outcomes.
It has a patient education element to make the patients the prime actors in their own care. It has a telemonitoring element to provide real time personalised information about the current state of heath of the patients.
The central concept of the Service is the productive and trusted interaction between the doctor and the patient.

Metabolic syndrome is also called metabolic X syndrome, syndrome X and also insulin resistance syndrome. Most healthcare offices should have a tape measure if not you can easily check your waist measurement at home. It is diagnosed after your healthcare provider has obtained more than 3 readings of an elevated blood pressure.

Hermida, PhD, director of the Bioengineering & Chronobiology Laboratory at University of Vigo, Spain. Therefore successful prevention for leg ulcers and successful treatment for leg ulcers must be directed at correcting the underlying cause, not the ulcer itself. A diagnosis is determined by the patient’s medical history, a thorough physical examination by a wound specialist or physician, and laboratory tests, which may include X-rays, MRIs, CT scans and noninvasive vascular studies to help develop a treatment plan. Ulcers in the elderly affect their quality of life, especially if they are affected by them chronically. Since many elderly individuals spend a majority of their time sitting, one of the best remedies is to keep their legs elevated, preferably above the heart. The failure of these valves causes blood to improper flow of the veins, causing varicose veins. Decreased circulation from diabetes is the main reason for the development of diadetic leg ulcers.
Treatment may involve wound cleansing, anti-inflammatory treatment and application of dressings. Leg and foot ulcers have a tendency to recur in elderly people, and sometimes may require years of therapy.
Under the Affordable care Act, re-admission penalties in the US have encouraged service providers to develop supportive discharge services which include Remote Patient Monitoring Services. The most common diseases where Remote Patient Monitoring has proven outcomes include CHF, COPD, Diabetes, Hypertension, Stroke, Chronic Renal Failure, but newer innovative programs will support Speciality Treatment Programs. Patient education programs can address the issue of non-compliance and support better adherence to therapy.

Deliver quality care at a lower cost, a critical imperative in the accelerating era of value-based payment. Enable more timely intervention from caregivers and clinicians through near real-time data monitoring and alerts, reducing ER visits, hospital readmissions and inpatient days. Increase behavior change and care plan compliance (compliance with lifestyle changes, such as diet, exercise and medication).
Dramatically reduce or eliminate “false positive” alerts and the need for clinicians to follow-up on minor irrelevant issues.
Receive alerts and regularly scheduled updates in the most convenient and relevant manner for each caregiver or clinician. Access robust reporting through highly secure portals to obtain actionable data, advanced analytics and reporting.
Make use of powerful population-level data reporting to provide authorised users with comprehensive patient data views. We enable our clients to implement robust, scalable, patient centred and integrated remote patient monitoring services that meet the needs of patients, their carers, healthcare professionals, providers and payers. National Health Service (NHS) conducted a randomised control trial where they remotely monitored more than 6,000 patients with chronic conditions using remote monitoring services. To date this service, called TelemonitoringNI, has delivered over 1 million patient monitoring days.
As a direct result of an ageing population, chronic disease prevalence and complexity has increased along with total health expenditure.
The service supports patients with 12 main chronic condition categories including COPD, Heart Failure, Stroke, Diabetes, Hypertension, Chronic Kidney Disease and all co-morbidity combinations. With some 40,000 patients managed by 400 Family Doctors, it is a new care model for chronic patient that places the family doctor at the core.. Care Coordinators proactively schedule appointments with most urgent patients and inform the patient’s doctor of the clinical reason for the visit. Over 90% of all websites do this, however, since the 25th of May 2011 we are required by EU regulations to obtain your consent first. The important thing is that these characteristics put you at an increased risk for developing certain diseases.
And it is because of this link that a lot of people with diabetes type 2 inevitably end up on blood pressure medications also. This pattern, known as non-dipping, which can also occur in those with hypertension, is linked with glucose intolerance and other metabolic conditions. Evidence suggests, however, that these drugs work synergistically with other types of hypertension drugs including ACE inhibitors and ARBs, he said, which have been shown to reduce the incidence of diabetes. Many elderly individuals are inactive, making it very hard to treat ulcers in a traditional manner. Treatment for leg ulcers greatly depends on the factors that cause the ulcer or have prevented healing. So long as there is no arterial disease, venous leg and foot ulcers will benefit from elevation and compression dressings. Patients often have more than one chronic condition so support for co-morbidities is crucial.
The study showed that mortality rates decreased by 45% through the use of remote monitoring when compared with usual care.
They recognised the need to build a capacity to cope with the anticipated demand for services.
Growing evidence suggests that non-dippers face a variety of increased cardiovascular risks.
The two disorders have common etiologies and disease mechanisms including obesity, inflammation, oxidative stress and insulin resistance. Following an extensive review of the evidence base, it was decided to commission a Remote Patient Monitoring Service.
These interactions give the doctor and the patient the best chance to intervene in the disease progression.
When the arteries in the heart get clogged up with plaque, this is called coronary artery disease. If the plaque collects in the blood vessels in the brain, it is called cerebrovascular disease and can cause a stroke.
