The deepest hip flexor is the psoas, also known as the iliopsoas as it is often called in conjunction with the smaller muscle, the iliacus, which is on the inside of the Ilium (Iliac fossa). The iliopsoas is a key muscle not only for dancers to achieve the lift in their jumps, but also for athletes in running or jumping. When the iliopsoas gets into a shortened state, also known as ‘locked short,’ it will take the pelvis into an anterior tilt. Two of the primary causes of a locked short psoas are (a) sitting for long periods of time or (b) over exercising and not stretching properly.
Structural Integration and myofascial bodywork can greatly help with lengthening of the iliopsoas by separating the fascia (the connective tissue) in between muscle groups, breaking up adhesions in the fascia and adhesions binding to muscle or bone and lengthening taut fascia along the iliopsoas line. A structural integrator will use a system of usually 10-12 one-hour long sessions to change dysfunctional postural patterns in the body by manipulation of fascia and taut muscles.
To allow the new functional pattern to “hold” and prevent further discomfort, it is also important to lengthen the psoas.
To properly change the dysfunctional pattern of locked short psoas, we also need to strengthen the core muscles.

In the case of local pain the bone, muscle, ligaments and pockets of fluid, known as Bursa, are involved.
This can be very mild, causing discomfort on prolonged walking, to the severe, where the patient has difficulty getting out of a chair. I just got done doing my overhead pulley exercises on my arms and then I put on the wrap and it felt so good.
You will feel this irritation, swelling and inflammation when there is a strain, damage or micro-tearing to the tendon tissue. Over time the tendons in the hip will wear down resulting in something called degeneration.
In many cases, degeneration of the gluteus medius tendon is associated with greater trochanteric bursitis; the combined injury is called greater trochanteric pain syndrome. You may also experience swelling, warmth or redness around your injury and the pain often gets worse by moving your hip.
During a physical examination of your injured hip, your doctor will be looking for weakness and pain in the hip area. Only your doctor can give you a proper diagnosis and from this, determine a course of proper treatment. Conservative treatment options for hip tendinitis typically includes rest, icing of the injury, strengthening exercises, avoiding activities that cause or worsen the pain that you're feeling and anti-inflammatory over-the-counter medication (reference: 1, 2). Obviously, it delays the healing process, but what's worse is that every re-injury and additional healing cycle increases the amount of scar tissue that builds up around your original tendinitis injury.
The more severe your hip tendinitis is, the more likely that this scar tissue will make your tendon much more prone to injury again later on. Severe injury to the tendon (ruptured tendons) will require surgery to re-attach the tendon to your bone or bring the edges of the tear together. Anyone in the health-care business knows that your blood supplies the oxygen and much needed nutrients required to heal hip tendinitis injuries.

I was in the intensive care unit for 7 days and then in the hospital for 7 more before I was able to go home. Every muscle above and under the hips will have to respond automatically to every distortion or change in the location of the hips. In the most common bodily distortion, the right hip is rotated forward approximately ¼ inch to 1 inch more that the left hip. This is not because of an anatomical discrepancy in leg length, but because the right femur (thigh bone) extends from the right hip which, in our subject, is rotated forward. Next, the nervous system will recruit muscles between the left shoulder and neck to pull the head back to center to level the eyes. This muscle is the main culprit of lower back and hip pain in our society, since we have become very sedentary.
Since 90% of the time during walking the body weight is carried by one hip, the hip muscles are forced to work harder which make them susceptible to get in a spasm.
When they are spastic , pain would be felt in the hip and referred to the back and side of the thigh and calf. When they are spastic, pain would be felt in the low beck hip and would be referred to the back of the thigh. The psoas is attached to the lesser trochanter of the femur on the inside of the upper leg, and it attaches to vertebrae of the lumbar spine, L1 – L5, and T 12 in the lower back.
Proper core strength is also important for a balanced psoas, so weak abdominal muscles as well as weak back extensor muscles contribute to this dysfunctional low-back pattern. Expect 3-5 sessions to see lasting improvements and 10 – 12 sessions to permanently change a dysfunctional pain pattern in the low back and core due to tight psoas.
The SI practitioner will start building support from the feet and legs into the pelvis, core and back, and finally the neck by putting the head on “top”. In the next clip, we will see examples of how to stretch the psoas with active isolated stretching or AIS. It’s difficult for many people to strengthen abdominal muscles without causing strain on the lower back and neck.
When I talk about psoas’ tightness causing pain in the low back, I am also talking about the fasciae layer around the muscle and the surrounding bones. When chronic gluteus medius tendinitis occurs, the gluteus medius tendon is vulnerable to subsequent injury and degeneration that may result in a tear. If this sheath is injured it can also become irritated resulting in similar symptoms as tendonitis (pain, swelling and inflammation). Weakness in your hip and surrounding muscles and loss of range of motion while moving your leg away from the body are also some common symptoms of this condition.
Your hips should be at the same level and they may also look at the results of something called the 'Trendelenburg's Test'.

Over time the small tears and degeneration of the gluteus medius tendon is related to greater trochanteric bursitis. The more scar tissue that develops, the more you lose the range of motion in your hip and leg.

To enable the body to function in the most efficient way the hips have to be leveled horizontally. This occurs because there is frequently a tendency to use the right leg more than the left leg, for instance, as in the operation of an automobile with automatic transmission.
This change in head position will involve the righting reflex, which will be called into action to reestablish the eyes on the horizontal plane. While the subject is standing, his right hip will be higher than the left hip, and his left shoulder will be higher than the right shoulder. This muscle is the most used and abused in the body, because we use it all the time, while walking, standing and sitting. When the piriformis is in a spasm it will compress the thigh bone into the hip socket increasing the pressure on the joint. Without adequate length in the psoas, one cannot walk tall, and one can experience a hunched back and a lack of movement in the hips and legs.
The hamstrings will get overstretched, or ‘locked long.’ There will be an increase in lordosis in the lumbar spine, which will, in turn, affect the alignment of the whole spine and can cause both forward head posture and pain between the shoulder blades.
In the following clip we will see the location of psoas and an explanation of why it can cause low back pain. This is a stretching technique where one holds the end-range of the stretch for two seconds then comes out of the stretch; this is repeated 10-12 times. If left untreated, gluteus medius tendonitis can turn into a chronic condition or degeneration of your tendon fibers.
Chronic gluteus medius tendinitis can occur as the tissue is damaged and in a weakened state leading to partial or full-thickness tear of the tendon.
This is why it's so important to continuously use conservative treatment tools to heal any recurring tendon damage before it can build into something big. When this muscle gets tired and spastic, it will pull on the femur (thigh bone) onto the accetabulum (the hip socket) and increase the pressure on the hip joint. Also, when this muscle is spastic it will compress different nerves roots which exit the Sacrum (tail bone), including the "famous" Sciatic nerve.
What many do not realize is that much lower back pain can be traced back to the muscles of the hip; that is, when the hip flexor muscles are overly tight, they can lead to pain in the lower back.
One could, in fact, say that without proper length in the psoas, one would be found lacking in both physical poise and grace. By holding for two seconds at the end-range, we avoid activating the stretch-reflex, a defensive mechanism our bodies have to prevent the tearing of muscle tissue. It’s important to bring the navel in towards the spine to properly engage abdominal muscles and to protect the low back in doing these exercises. Level 1 practitioners can administer the first 3 sessions and Level 2 practitioners have 6 sessions in their tool box. This year, Magnus began Structural Integration therapy at the beginning of my training season.
Sport injuries, causing ligament strain and inflammation of the bursa are also common, with pain on movement and disturbed sleep. There is, indeed, a relationship, between lower back pain and tight hip flexor muscles and this will be elucidated below. Together with the Iliopsoas, these are the muscles that lift your leg when you walk, run or ascend stairs. Make sure to avoid bouncing or ballistic stretching, as this may cause injury, and go slowly and breathe deeply.
These exercises will also strengthen the iliopsoas without causing shortness in the tissue. Ida Rolf said that fascia is the “organ” of posture, and it can be manipulated into letting go and freeing up space within the body, and thereby allowing for greater functional movement.

You're not supposed to "feel" a lot of heat because the heat isn't treating your skin, it's treating your hip increasing the blood flow right in your tendon tissue. Many people my age (55) don't even ever get in that kind of shape, let alone a year after a major accident with all the complications that I had. In the most severe cases the patient will be unable to lie or stand comfortably, and changes in position will not relieve the pain. Perhaps that may explain the “super human” strength that can occur in an emergency with the fight or flight response, but more about that some other time. Intense pain will occur when the patient sits or squats since this type of movement requires external rotation of the upper leg and bending the knee. Doctors explained to me that as we age tendons lose flexibility and are more prone to injury.
I bought that explanation to a point but again tried physical therapy, prolonged stretching, Ibuprofen and rest.
I had relief if I used large regular doses of anti-inflamatory medications but was concerned about the long term negative affects of the meds.
