Shoulder pain is an extremely common complaint heard by Elite Sports Medicine - Woodland Hills chiropractors.

Shoulder pain could be caused by joint dislocation, joint inflammation, muscle tear or muscle spasm. Upper Trapezius - The Upper Trapezius and Levator Scapulae muscles connect the cervical vertebrae to the shoulder blades (scapulae) and upper thoracic spine. Latissimus Dorsi - This muscle originates in the lower back and is connected to the inside of the shoulder. Supraspinatus - This muscle is originated in the upper part of the scapulae (shoulder blade) and is connected to the shoulder joint. Infraspinatus (Rotator cuff muscle) - This muscle covers the lower part of the scapulae (shoulder blade) and is connected to the shoulder joint from the back. Teres minor (Rotator cuff muscle) - This muscle is originated in the lateral (outside) edge of the scapulae and is connected to the back of the shoulder joint.
Subscapularis - This muscle is originated in the anterior (front) part of the scapulae and is connected to the front part of the shoulder joint.
Triceps and Anconeus - These muscles are connected between the shoulder joint and the posterior (back) forearm.
The acromioclavicular (AC) joint in the shoulder is a common spot for osteoarthritis to develop in middle age. The shoulder is made up of three bones: the scapula (shoulder blade), the humerus (upper arm bone), and the clavicle (collarbone).
The part of the scapula that makes up the roof of the shoulder and connects with the clavicle is called the acromion. However, the AC joint is different from joints like the knee or ankle, because it doesn't need to move very much. AC joint osteoarthritis may also develop following an injury to the joint, such as an AC joint separation. In its early stages, AC joint osteoarthritis usually causes pain and tenderness in the front of the shoulder around the joint.
Initial treatment for AC joint osteoarthritis usually consists of rest and anti-inflammatory medications such as aspirin or ibuprofen.
The older open method of performing this operation is done by making a small incision, less than two inches long, over the AC joint. If you don't need surgery, range-of-motion exercises should be started as pain eases, followed by a program of strengthening. Therapy goes slower after surgeries where an incision is made through the shoulder muscles. Some of the exercises you'll do are designed to get your shoulder working in ways that are similar to your work tasks and sport activities.
Other types of shoulder joint fracture do not end up in compound fracture unless injury is severe and associated with multiple body injury.

Acromion Fracture- Acromion bone (part of scapula) forms an Acromio-clavicular joint with clavicle bone. Coracoid Fracture- Coracoid is a protrusion of scapula adjacent to Gleno-Humeral and Acromio-clavicular joint. Severe to very severe pain is observed in fracture of humerus, fracture of neck of scapula and glenoid socket fracture.
Severe to very severe pain follows fracture of humerus, neck of scapula and glenoid socket fracture. Shoulder joint swelling is caused by post-traumatic soft tissue edema and hematoma (blood clot). Protrusion of broken (fracture) end of the bones through soft tissue causes uneven hard "bump" of shoulder joint. Patient is unable to move shoulder joint because of muscle spasm and disruption of joint link.
Conservative treatment with NSAIDs is often adequate to treat hairline fracture and non-displaced fracture of the shoulder joint.
Physical therapy is advised while shoulder joint movement is restricted with sling and rest.
Shoulder joint replacement surgery involves replacement of head of humerus and glenoid socket with plastic or metal prosthesis. Pain often continues and is felt at severe intensity after complete healing of fracture site because of muscle attachment getting scarred following injury and during healing of the fracture. Muscles end or tendon is trapped in calcified bone around fracture site and muscle contraction causes severe pain. They could happen because excessive activity of muscles, a fall or a blow to the shoulder as in a car accident. While in a spasm they will produce pain to the back of the neck, to the shoulder and headaches. When in a spasm the pain will be felt in the anterior part of the shoulder and the lower back. When in a spasm the pain would be felt in the shoulder itself with referred pain down the arm to the elbow. When in a spasm the pain would be felt in the shoulder and all the way down the arm to the wrist.
When in a spasm the pain would be felt between the scapulae and the spine and down the arm all the way to the fingers.
When in a spasm the pain would be felt in the side of the shoulder and in the back of the forearm. When in a spasm the pain would be felt the top and back of the shoulder and down the inner part of the arm and the wrist. When in a spasm the pain would be felt in the shoulder, chest and down the inner part of the arm to the elbow area.
When in a spasm the pain would be felt in the anterior (front) shoulder, and down the arm to the fingers. When in a spasm the pain would be felt in the shoulder and the back of the arm and forearm.


Degeneration of the AC joint can be painful and can cause difficulty using the shoulder for everyday activities. The pain is often worse when the arm is brought across the chest, since this motion compresses the joint.
X-rays can show narrowing of the joint and bone spurs around the joint, which are signs of degeneration. The AC joint is very close to the surface of the skin and can be easily reached through a small incision. Treatments start out with range-of-motion exercises and gradually work into active stretching and strengthening.
Active range-of-motion exercises help you regain shoulder movement using your own muscle power. Exercises will focus on improving strength and control of the rotator cuff muscles and the muscles around the shoulder blade. Your therapist will help you find ways to do your tasks that don't put too much stress on your shoulder. This integrated approach helps determine the best and quickest shoulder pain treatment protocol for you. After locating the misaligned joint in your body, we manually apply gentle pressure and reposition the joint to its proper joint mechanics. The anterior and lateral scalenes could compress nerves, and will produce pain and radiation (tingling sensation and numbness) to the neck, shoulder, and arm down to the fingers. When in a spasm, the pain will be felt in the front part of the shoulder, the chest and down the inner arm to the fingers. The pain is vague and may spread to include the shoulder, the front of the chest, and the neck. This leaves a space between the acromion (the piece of the scapula that meets your shoulder) and the cut end of the clavicle, where the joint used to be. The program advances to include strength exercises for the rotator cuff and shoulder blade muscles.
The first few therapy treatments will focus on controlling the pain and swelling from surgery. These chiropractic adjustments can help restore alignment, improve shoulder mobility, relieve shoulder stiffness and provide sustainable shoulder pain relief. The muscle fibers will start to compress blood vessels, compromise blood circulation, entrap nerves, and compress joints.
The shoulder does heal, but many years later degeneration causes the AC joint to become painful.
If the joint has been injured in the past, there may be a bigger bump over the joint on the affected shoulder than on the unaffected shoulder. Your surgeon will take care not to remove too much of the end of the clavicle to prevent any damage to the ligaments holding the joint together. The goal is to get your shoulder moving smoothly and to learn how to control your symptoms. Shoulder joint fracture includes fracture of proximal (close to shoulder joint) part of humerus, glenoid cavity, acromion, coracoid process, neck of scapula, body of scapula and clavicle bone. This may result in less damage to the normal tissues surrounding the joint, leading to faster healing and recovery. Your therapist may also use massage and other types of hands-on treatments to ease muscle spasm and pain. Fracture of the lateral (outer- near shoulder joint) end of clavicle is the most common shoulder joint fracture observed in children.
